We are seeking a Think Family Worker with the skills and experience to assess and engage with service users, ensuring they access the right interventions at the right time, tailored to their needs.
This role involves supporting the Great Yarmouth team to identify service users with complex safeguarding and family involvement, including pregnant individuals, and creating tailored care plans.
Flexibility, curiosity, and good listening skills are essential, as no two days are the same and every person is treated as an individual.
Where: Great Yarmouth, Norfolk
Hours: Full Time, 37.5 hours per week
Salary: £27,861.26 - £32,002.35 (Pro Rata, depending on experience)

About the role
* Support social (re)integration of service users to promote recovery, resilience, peer support, and self-determination.
* Manage caseloads involving individuals with complex safeguarding needs, including pregnant individuals and those with children involved in social care.
* Follow safeguarding practices and collaborate with statutory agencies.
* Provide assessments, psychosocial interventions, care planning, and aftercare support.
* Advocate for service users to facilitate engagement and access to partnership services.
* Develop strong relationships with partner agencies, ensuring effective referral protocols and care pathways, especially for 'hidden harm' interventions.
* Represent the service at partnership meetings such as MARAC, child in need, and child protection meetings.
About you
* Strong understanding of substance misuse issues and relevant experience.
* Experience working with service users facing multiple risks like domestic violence, unemployment, poor housing, pregnancy-related harm, etc.
* Experience collaborating with social services and safeguarding boards.
* Proactive, independent, with excellent time management and prioritization skills.
* Knowledge of mental health interventions and services.
* Supportive team player with strong interpersonal skills and partnership working abilities.
* Ability to manage change effectively, prioritizing service user needs.
* Understanding of information governance and safeguarding procedures.
* Experience conducting assessments, care planning, case management, and reviews.
* Knowledge of Child Protection procedures.
