Pay, Bonuses & Rewards
* Salary: £27,849 per annum + generous on-call payments
* Hours: 37.5 hrs per week (9am–5pm)
* Contract: Full-time, permanent
* Location: Newcastle Office (Gosforth)
* On-call: Rota basis, including some weekends (additional pay)
* Driver: Essential
* Bonuses: Up to £300 new starter bonus, £300 Refer a Friend bonus, and £400 annual bonus

The Role – What You'll Be Doing
As a Care Co-ordinator, you will plan and organise support so that:
* Colleagues have clear, consistent runs and rotas
* Travel is minimised, so more time is spent with the people we support
* Care is delivered by familiar, well-trained teams, maximising continuity
You will:
* Coordinate and schedule support calls, runs and rotas
* Create efficient runs, minimising travel while meeting everyone's needs
* Prioritise your workload, staying calm under time pressure and acting proactively to prevent concerns and complaints
* Use information and data to inform your understanding and decision-making
* Respond to complaints and concerns in a timely, constructive way, helping to resolve issues early
* Communicate compassionately and clearly with:
o The people we support and their families
o Colleagues
o GPs, District Nurses, Social Workers and other professionals
Coordination of Support
You will be responsible for:
* Creating consistent runs and rotas, matching customers and colleagues to maintain high levels of continuity
* Ensuring support calls are covered in advance, minimising the impact of unplanned absence
* Working with the Trainer to schedule training and shadowing, providing cover for calls or adjusting time to maintain training compliance
* Contact logs and communication records on our systems
* Maintaining accurate, up-to-date records for:
o Care calls – ensuring they meet the needs, wishes and preferences of the people we support
o Colleague information – availability, absences, location and travel, so you know who is available each day
Support Quality & Customer Satisfaction
A key part of your role is to:
* Communicate with the people we support, their families and colleagues about new services and changes to support
* Build and maintain professional relationships with people we support, families, colleagues and external partners
* Respond promptly to concerns and issues, intervening early and resolving problems before they escalate
* Keep colleagues informed and request support when needed, so we work as one team
About You
We are looking for an exceptional and credible candidate with:
* Experience of coordinating and scheduling (care coordination experience is desirable)
* Proven ability to prioritise work, stay focused under pressure and manage competing demands
* Strong verbal and written communication skills
* A collaborative approach – using information, data and feedback to inform discussions and decisions
* A driver, able to travel as required
