Job overview

If you are a qualified District Nurse or an enthusiastic experienced Community Staff Nurse looking for a development opportunity to expand your knowledge and skills in leading and supporting staff, this may be the ideal opportunity for you.

The successful candidate will have a can-do attitude and be responsible for assisting in the management and leadership of the Integrated Care Nursing Team. You will support the on-going development and transition of the community nursing service.

You will need to have some management and leadership experience and possess excellent interpersonal and communication skills, with the ability to work within a complex and dynamic work environment. Working with an extensive multidisciplinary team, you will be supported by and work closely with the District Nurse Team Leader and other members of the primary and community health care Due to the requirement to undertake visits the post holder is required to be a car driver with a valid driving licence.

Main duties of the job

Main duties of the job

1. In collaboration with the Team Leader assist in the leadership, management and supervision of the Community Nursing Team, facilitating research and evidence based practice.
2. To lead the assessment process and maintain high quality effective nursing care in the community
3. To support case management of complex patients within the wider team
4. To support Multi Disciplinary Team working in collaboration with key partners/commissioners/GPs
5. To support the Team Leader in ensuring service delivery is in line with the Core Offer and the
Standard Operating Procedures of the Community Nursing Service

Qualifications/Training

Essential criteria

6. Registered Nurse Level 1
7. District Nurse degree or equivalent. (or a willingness to complete DN Qualification as directed by management)
8. Experience of administration of Intravenous medications
9. Mentorship qualification
10. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.

Desirable criteria

11. Nurse prescriber (V150)
12. Non-Medical Prescriber (V300)

Experience

Essential criteria

13. Experience of working with; Older people, Palliative Care, chronic disease
14. Continuing professional knowledge and development.
15. Experience of teaching and mentorship.
16. Evidence of knowledge and experience of Case Management.
17. Change management and leadership skills.
18. Working in partnership with other healthcare providers, Social services etc.
19. Experience In the management of staff
20. Experience of multidisciplinary working

Desirable criteria

21. Mental Health Experience
22. Experience of multidisciplinary team working and Case Management models

Knowledge

Essential criteria

23. Sound understanding of Primary care and multi agency working.
24. Awareness of the value of audit/research and evidence-based practice
25. Awareness of NHS issues

28. Evidence of ability to communicate both verbally and in writing at all levels in multi-disciplinary forum and with users and carers.
29. Able to organise and coordinate services in collaboration with, and in the absence of the District Nurse Team Leader

30. Due to the requirement to undertake visits and transport non-portable equipment, the post holder is required to be either a car driver with a valid driving licence or have access to an alternative means of transport.
