Job overview

Oldham Early Intervention Team are looking to recruit a band 6 Care Coordinator for a fixed term contract until 31/12/26 to cover for secondment.


Main duties of the job

To act as Care Coordinator for service users identified as having a first episode psychosis, being in the early phase of a psychotic illness or at risk mental state. To assess needs and deliver interventions in line with currently identified best practice in line with NICE guidance. To provide advice and supervision as required to other mental health professionals within the multi-disciplinary team and to contribute to ongoing service developments.

Detailed job description and main responsibilities


Main Duties and Responsibilities Clinical

• To act as a Care Coordinator carrying a complex caseload, meeting the criteria for the service delivering comprehensive support for identified needs.

• To identify the needs of service users by utilising Trust approved research validated assessment tools.

• To employ an assertive outreach and flexible approach as and when necessary to increase levels of engagement.

• To liaise with all appropriate statutory and non-statutory agencies as required in relation to the service user’s needs.

• To implement and manage co working strategies as necessary within the multidisciplinary approach

• To carry out interventions in line with the psychosocial approach of the overall service focussing on recovery and re: enablement.

• To ensure carer assessments are carried out and identified areas of need are addressed.

* Engage in family work where needed and provide psycho social education and information as required.

• To carry out physical health screening and intervention in line with NICE guidance (training provided)

• To identify risk issues and implement and evaluate risk management plans according to policy and procedure.

• To be accountable for ensuring that within the resources at your disposal all service users receive a high quality service.

• Meet Trust standard CPA requirements

• Ability to formulate and deliver care to complex clients and their families


Managerial

• To take a lead clinical role within the team and in the absence of the manager / deputy manager deputise on their behalf.

• Offer clinical supervision to staff within the team, and ensuring that the practice of workers is of a consistently high standard and reflects the protocols and guidelines developed both locally and nationally.

• To have an overview of the services needs and priorities and take responsibility for prioritising action as needed.

• To take an active part in governance and service development.

• To take responsibility for organising a response to the care needs of other care coordinators service users in their absence.

• To liaise with and report to the senior manager as required.

• To contribute to the development, evaluation and monitoring of the operational policy, procedures and protocols for the service.


Teaching, Supervision and Training

• To receive regular clinical and managerial supervision

• To contribute to the learning environment for any students or workers on placement with the team.

• To support colleagues in areas of professional development relevant to the improvement of the teams service delivery.

• To provide a mentoring role to students and new staff.

• To contribute to training programmes in a variety of settings.

• To provide clinical supervision.

• To develop skills in accordance with service need and registration requirements.  To contribute to Borough and Trust wide training initiatives.


Research and Service Evaluation

* To contribute to the provision of data for service evaluation.
* To ensure that clinical interventions carried out by self and trainees/supervisees are informed by reliable and up to date research and that service users are offered treatments with the best-proven efficacy.


Person specification


Educatoin/Qualifications


Essential criteria

* Registered Mental Health Nurse (RMN / Diploma / Degree), Occupational Therapist or Social Work registration
* Qualification to post graduate level or equivalent worked experience relevant to the role.


Desirable criteria

* Approved Mental Health Practitioner.
* Mentorship / Practice Leadership qualification.


Experience


Essential criteria

* Experience of giving clinical supervision
* Experience of working effectively in a multi-disciplinary team.
* Experience working with clients with serious mental illness and their families/carers.
* Experience of care co-ordinating complex packages of care.
* Experience of conducting high quality risk assessments and implementing risk management plans


Desirable criteria

* Experience in early intervention for psychosis, e.g. specialist placement, previous employment in the early intervention service, participation in a research trial or personal experience.
* Experience in the use of psychosis-specific semi-structured interviews (e.g. PANSS; CAARMS; PSYRATS).
* Experience of managing Community Treatment Order’s.


Knowledge


Essential criteria

* Knowledge of the ethos and philosophy of early interventions to support their clinical practice.
* A high level of knowledge and around Early Intervention for Psychosis


Desirable criteria

* Knowledge of research in psychosocial interventions and clinical implication
* A high level of knowledge about the evidence base and guidance for interventions for those with psychosis and an At-Risk Mental State
* Knowledge of NICE guidelines for psychosis and schizophrenia.


Skills and Abilities


Essential criteria

* Able to organise, plan and implement own work and oversee that of others.
* Able to work to set workload deadlines and to coordinate staff to achieve same.
* To be self-motivated and to manage time effectively
* An ability to interact appropriately and professionally with colleagues.
* Excellent interpersonal skills.
* Excellent verbal and written communication skills.


Desirable criteria

* Experience of delivering service presentations and liaison to outside agencies to encourage referrals.


Work related circumstances


Essential criteria

* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
* Willing to carry out all duties and responsibilities of the post in accordance with the Trust’s Equal Opportunities and Equality and Diversity policies
* Appointments to regulated and controlled activities require an enhanced DBS disclosure.
