With an emphasis on patient‑centred and community‑based care, you will work alongside occupational and physiotherapists to help provide access to cognitive behavioural therapies (one‑to‑one or group) and support colleagues in delivering management interventions. Responsibilities
* Deliver excellent patient care along a care pathway across organisational boundaries, supporting senior colleagues to deliver individualised treatment plans for patients with complex respiratory conditions.
* Work with allied health professional colleagues to deliver the symptom management programme in 1‑to‑1 and group settings.
* Monitor and assist with equipment provision as part of patient care.
* Provide flexible working arrangements and contribute to a supportive, resourceful team environment.
Additional Role Details
* Role size: 22.5 hours per week; base locations across East Kent; role works across all areas and may require travel. Driving and access to a car are required if driving is a role component.
* Working pattern: 22.5 hours between Monday and Friday; pattern to be agreed.
* This community trust emphasises vaccination and continuous support to ensure safety and wellbeing for patients and staff.
Qualifications and Requirements
* Full training and induction provided; ability to work across care pathways with respiratory patients.
* Willingness to travel across East Kent; may require driving; a full UK driving licence is requested where driving is a requirement for the role.
