Overview
We are a large PCN of 11 practices serving over 103,000 patients. This is a practice-based opportunity with our PCN, employing the majority of Pharmacists and Pharmacy Technicians directly to support learning and development under the leadership of an experienced Lead Clinical Pharmacist. This is not a remote role.
Main duties
To participate in the provision of a Clinical Pharmacy Service to our member GP Practices; including routine clinical medicines reviews and reducing inappropriate poly-pharmacy and wasteful prescribing through timely clinical medication reviews. Reconcile patients’ medicines after hospital discharge and collaborate with patients and community pharmacists to ensure access to needed medicines post-discharge. Liaise with community and hospital pharmacy colleagues and develop referral processes between primary care professionals, promoting the repeat dispensing service. Manage the repeat prescribing reauthorisation process by reviewing requests and medicines approaching review dates. Handle patient and health care professional medicine queries. Work with primary care professionals and patients to implement NICE and other evidence-based guidelines. Run clinics as appropriate, contribute to multi-morbidity and multidisciplinary reviews, and provide medicines information and training to PCN healthcare professionals and admin staff. Act as a source of medicines information for the practice team and patients (e.g. dosing, side effects, adverse events, possible alternatives, out-of-stocks).
Act as PCN point of contact for all medicines-related matters, establishing positive working relationships. Ensure patient safety during transfers between care providers through reconciliation of prescribed medicines. Provide education on medicines for the clinical team and review the latest guidance ensuring PCN conformity with NICE, CQC, etc.

Key responsibilities
1. Work within the Primary Care Network team to undertake medication reviews, particularly in high-risk groups (e.g., frail elderly, polypharmacy, renal impairment, hepatic impairment, substance misuse, high-risk medicines, STOPP/START identified patients, revolving door hospital admissions).
2. Improve prescribing practice through educational support for all prescribers within the PCN.
3. Lead on changes in prescribing across patient populations when evidence changes (e.g., drug withdrawal or changing indications).
4. Liaise with community pharmacy colleagues to align support for medicines adherence.
5. Support improvements in clinical care through practice-based audits and implementation of change.
6. Provide prescribing advice to prescribers within the PCN (e.g., temporary non-availability of drugs).
7. Ensure patient safety when transferring between care providers through reconciliation of medicines.
8. Provide additional duties as requested: support patients with self-limiting conditions, encourage cost-effective prescribing, review medications for newly registered patients, implement robust repeat prescribing and electronic repeat dispensing, re-authorise repeat prescriptions within scope, identify patients suitable for repeat prescribing, support management of drug and alcohol dependencies, signpost patients to appropriate healthcare professionals, assist with QOF targets and LTC review clinics, administer vaccines when requested, educate the clinical team on medicines, review guidance to ensure NICE/CQC compliance, and act as PCN’s medicines liaison.
Qualifications, experience and other requirements
Qualifications
* Undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council
* Minimum 2 years post-graduate experience in pharmacy, with at least 1 year in general practice
* Member of the Royal Pharmaceutical Society (RPS)
Desirable
* Prescribing qualification; postgraduate pharmacy qualification or progression; awareness of common conditions in general practice; RPS Faculty membership
Other
* Full driving licence and own car (travel between practices)

* Knowledge of EMIS
Experience
* Experience of NHS/PCN service delivery; strong influencing and relationship skills; ability to work in a multi-disciplinary team; effective communication with clinicians, patients and the public

* Experience supervising staff and empowering clinical professionals to improve cost-effectiveness and quality of care

We are a large and diverse PCN covering Nuneaton and Bedworth in North Warwickshire, with over 60 ARRS staff across our locality, including Care Coordinators, Social Prescribers, Health & Wellbeing Coaches, Mental Health Practitioners, First Contact Physiotherapists, Dietitians, Physician Associates and Nurse Associates alongside Clinical Pharmacists and Pharmacy Technicians. We have a dynamic Senior Management Team supported by a Digital and Transformation Team, with initiatives including Rapid Health telephone triage, Sleep Station, Pure Health & Wellbeing pain pathways, Pain Cafes, menopause drop-in clinics, and a 12-week health and wellbeing programme with Sky Blues in the Community. The team includes Pharmacists and Pharmacy Technicians at varying career stages, led by a Lead PCN Clinical Pharmacist. We value staff development and training. The majority of Pharmacists work across two member practices.
