This role will be central to the co-design and delivery of new assessment models, working closely with system partners and community stakeholders to improve awareness and access—especially for under-represented groups.

Essential Criteria:
Registered healthcare professional with ACP qualification.
Experience in dementia care or memory services.
Strong commitment to reducing health inequalities.
Excellent communication and stakeholder engagement skills.
Ability to work autonomously and collaboratively in a multi-agency environment.
