Wellbeing Worker - Integrated Disability Service (IDS)

Job description

About the role:
To undertake work with children and families as the sole allocated worker under the direction of the Team Manager. The family will have been the subject of an assessment by a Social Worker and the level of need of the children will have been determined as being a need for general support with no risk of immediate harm. There will be a plan drawn up by a Social Worker for the work to be undertaken with the family by the
Wellbeing Worker in order to intend that the situation will resolve and the case can be closed. There may also be other work for families who also have an allocated Social Worker. There may be a need for flexible working during the evening and at weekends. To undertake this work, the Wellbeing Worker will need to do direct work with children and parents, and may call upon the resources of the Family Support Team and Early
Help in order to support the plan.
About you:
Wellbeing Workers will assist in ensuring that the Local Authority meets their statutory obligations under current legislation and will have a knowledge/awareness about policies and procedures in relation to safeguarding and Welsh Legislation.
To work collaboratively as part of the Integrated Disability Service Team to improve outcomes for children and young people aged 0 – 18 years who have a learning disability, physical disability or ASD.
To work holistically alongside colleagues from Health and Education as part of the Integrated Disabilities Team to ensure that the assessed needs of children with a disability and their families are met through the Person Centred Planning and reviewing process in line with the All Wales Act. To promote equality and inclusion for all children and young people with a disability via the adoption of a strengths based approach to assessment and care and support planning.
What you will do:
Wellbeing Workers will provide support and assistance to stabilise and improve the current living arrangements for the child or young person.
