Job summary

An exciting and rewarding opportunity has arisen, for the post of Crisis Clinician within the Teesside Crisis North Home Intensive Treatment Service.

The focus of the team is to provide an alternative to in-patient care, focusing on people experiencing severe mental health difficulties that should be treated in the least restrictive environment with the minimum of disruption to their lives.

Nursing candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying.

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.

Main duties of the job

The team will work with medical colleagues in the ongoing assessment and intervention of existing clients and new referrals presenting in crisis to provide short-term intervention as an alternative to in-patient care. The team will liaise closely with community services, in-patient care and other locality based services as well as all relevant non statutory agencies to ensure a seamless provision of care.

The post holder will be responsible for the ongoing assessment of both new and existing service users ensuring all care is planned, implemented, reviewed and evaluated where required.

The post holder will demonstrate and deliver a wide range of clinical skills, practical procedures and nursing interventions, which will directly enhance the effectiveness of treatment to the individual.

The post holder will develop close, effective working relationships with personnel concerned with planning and delivering care to the defined group and ensure input into all multi-agency/disciplinary meetings and groups where appropriate.

Job description

Job responsibilities

Promotes and maintains safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs.

Takes steps to obtain patient consent to care and treatment in accordance with the Mental Capacity Act, ensuring that patients who lack mental capacity remain at the centre of decision-making and are fully safeguarded. Acts, wherever applicable, in accordance with the Mental Health Act and associated policies and procedures.

Responsible for recognising the potential for or signs of patient harm, abuse or neglect, including poor clinical practice, reporting all such concerns and taking all reasonable steps to protect the patient. Responsible for identifying and reporting concerns regarding the safeguarding of children who may be at risk.

Responsible for ensuring the highest professional standards and attitudes towards the care of patients are maintained at all times and that care is delivered in accordance with evidence based practice by all members of the team. Patients have a range of mental health conditions (and/or may present with learning disabilities at initial assessment), may demonstrate behaviours that challenge and may have difficult family situations of relevance, a history of child abuse etc.

Manages risk and provides care pertaining to the immediate situation and decides the appropriate course of action. short term home treatment provided by the Crisis Team, referral to community mental health teams or counselling services or admission to an inpatient unit.

Person Specification

Qualifications

Essential

1. Current professional registration with an approved professional body
2. Evidence of Continuing Professional Development.
3. Clinical Supervisor
4. Key skills in literacy, numeracy and ITQ level 2 (or equivalent
5. Recognised sign-off mentor or must be achieved within agreed timescale.
6. For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale
7. 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework)

9. Post-graduate qualification in a relevant area

10. Significant experience working with people with mental ill health in a community setting
11. Working collaboratively with service users and their families/carers.
