We are thrilled to present this exciting opportunity for two full-time Social Workers in the Community Mental Health Team within Adult Services, covering Swansea. This temporary role spans an initial three-month period with potential for extension. As part of this dynamic team, you will work 37 hours per week, with a blend of office-based tasks and client visits in hospitals and their homes. If you have mental health experience alongside the ability to prepare reports and participate in Mental Health Tribunals, then you might just be the perfect fit! Familiarity with WCCIS is a bonus, and local knowledge of Swansea or a willingness to travel would be ideal.

What you will do:
* Conduct assessments of service users within the community, hospitals, and at home.
* Develop and implement care plans tailored to each individual’s mental health needs.
* Write and submit detailed reports for service reviews and Mental Health Tribunals.
* Collaborate with healthcare professionals to deliver coordinated services.
* Provide advice and support to service users and their families.
