About the Role

We are part of a large integrated drug and alcohol service working with some of Cornwall's most excluded and disadvantaged people. Successful candidates will join the adult community team, working alongside in-house services including outreach, hospital, clinical, criminal justice, and residential rehabilitation facilities.

We have an exciting opportunity for a full-time Outreach Worker in our Cornwall services, based in Truro. This role involves community work across Cornwall from a trauma-informed perspective. A full driver's license and access to a car are required due to the outreach nature of this role.
The Hospital Outreach Service offers rapid response to individuals frequenting the Royal Cornwall Hospital in Truro with issues related to substance use.
The team collaborates within a multidisciplinary model with partners in safeguarding, housing, homelessness, domestic abuse, healthcare, and mental health services.
* Evidence of ongoing professional development & OCN/NVQ Level 3 or equivalent, or willingness to achieve within 12 months
* Full UK driving license and access to a vehicle
* Experience in health, social care, or related settings
* Successful experience working with substance misuse clients
* Understanding of trauma-informed practice
* Ability to manage own caseload
* Knowledge of safeguarding processes
* Understanding barriers to healthcare for people with substance use issues
* Ability to build and maintain effective relationships with clients and colleagues
* IT competence
* Strong oral and written communication skills
* Ability to work effectively in a team and support colleagues
