Job Summary

Healthcare Assistant

22.5 hours / 3 days (7.5 hour shifts)

£12.8887 per hour with NVQ level 3
£12.3894 per hour without NVQ level 3

The role is part of the Hospice at Home day team. You will demonstrate clinical excellence and maintain consistently high standards of individualised care, compassion and bereavement support. The aim is to make living with dying better for patients and their families.

About Pendleside Hospice

Pendleside Hospice was founded as a charity in 1988 and is one of the principal providers of palliative and end‑of‑life care services to adults in Burnley and Pendle. All our care is free of charge to the patients and their families/carers, funded primarily by the generosity of the local community.

Job Description

* Assist to provide high quality care to people with palliative needs in their own homes.
* Support and contribute to an environment of continuous quality improvement and learning.
* Adhere to hospice policies, procedures and standards, especially confidentiality, information governance, health and safety, and infection control.
* Work independently and with nursing colleagues to deliver individualized, skilled care and report changes in condition to the appropriate healthcare colleague.
* Prioritize and plan workload according to individual need in partnership with the team and multidisciplinary teams.
* Work autonomously with defined workload and indirect supervision from the Hospice at Home shift coordinator or registered nurse.
* Implement and review patient care plans according to Hospice policies and procedures.
* Provide family and bereavement support to relatives and carers, demonstrating a realistic attitude towards people facing a life‑limiting diagnosis.
* Communicate effectively and collaborate with members of the multidisciplinary team, ensuring coordinated service and positive working relationships.
* Address questions, concerns and complaints from patients, families and carers in line with Hospice policies and procedures.
* Maintain accurate, up‑to‑date patient records in accordance with Hospice policies and procedures.
* Implement health and safety procedures and highlight any risks to patient or staff safety for action.
* Enforce infection control measures and report any risks immediately.
* Record and report all accidents, complaints and untoward incidents as per Hospice policies.
* Apply standards relating to patient assessment, admission and discharge consistently.
* Ensure documentation is completed accurately and fully in line with Hospice record‑keeping policy.
* Monitor patients’ clinical condition, identify significant changes and adapt care accordingly.
* Utilise IT skills, including Microsoft Office, email, internet and the hospice patient record (EMIS).
* Communicate effectively with distressed and/or bereaved individuals sensitively.
* Manage situations where people are distressed or angry safely.
* Communicate clearly with people exhibiting challenging behaviours, respecting them while setting clear expectations.
* Maintain excellent telephone communication skills, managing calls from patients, carers and professionals sensitively.
* Handle complaints sensitively in compliance with Hospice policy.
* Identify communication problems with the hospice or external agencies and report promptly.
* Advocate for service users ensuring their views are known, including supporting advance care planning discussions and documentation.
* Maintain confidentiality at all times.
* Develop and maintain relevant knowledge and skills; be responsible for own continuing professional development.
* Attend and complete mandatory training and other essential training relevant to the job role.
* Provide mentor support to new staff, apprentices, students and volunteers.
* Identify and participate in the development of teaching and educational programmes appropriate to your role.
* Participate in one‑to‑one supervision with own line manager.
* Contribute to organisational service development and participate in quality improvement activities such as audit, research, evaluation, user involvement, and educational programmes.
* Take an active role in unit meetings to highlight areas requiring quality improvement.
* Use and maintain resources efficiently and encourage others to do so.
* Identify and carry out risk assessments, taking immediate remedial action and updating them regularly.
* Adhere to adverse event and serious incident reporting policy.
* Support implementation of safeguarding policies, the Mental Capacity Act and Deprivation of Liberty Safeguards.
* Promote an environment that supports infection prevention and control policies and procedures.
* Use equipment safely and report faults as necessary.
* Ensure compliance with Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 and contribute to the Care Quality Commission inspection process.
* Monitor service delivery outcomes in line with regulatory requirements.

Health & Physical

* Good health and attendance record.
* Ability to move and handle people and equipment.


Qualifications

* Evidence of recent appropriate study/development and application in the work environment.
* Commitment to ongoing training and personal development.
* Desirable: Level 3 in health and social care.
* Desirable: Palliative and/or end‑of‑life care training.


Disposition / Attitude / Adjustment

* Evidence of ability to work independently and as part of a team.
* Demonstrated understanding of confidentiality.
* Commitment to the development of the role of Health Care Assistant.


Experience

* Essential: Experience of working with adults.
* Essential: Experience of working with adults with end‑of‑life and/or palliative care needs.
* Essential: Ability to relate experience to a hospice and community setting.
* Essential: Understanding of the role of Health Care Assistant.
* Desirable: Experience of working within a health care setting.
* Desirable: Experience of working in a palliative care setting.


Practical & Intellectual Skills

* Understanding and sensitivity to the needs of palliative care patients.
* Liaison with other agencies and professionals.
* Good communication skills.
* Ability to record clearly and effectively.
* Ability to undertake a range of patient observations and procedures.
* Full UK driving licence and use of own vehicle.
* IT/computer literacy.
* Desirable: Communication skills training.
