The post holder will play a key role across the CHC service, workingwithin the remit of the National Framework for Continuing Healthcare, todeliver an effective, efficient and equitable service to the people andpopulations of Sussex.
The post holder will need to have CHC experience together with excellentverbal and written communication skills, as you will be required to workclosely with patients, families, carers and other external partners.
Main duties of the job
As a Senior Practitioner, your role will include delivery of a varietyof functions across the CHC service, including: Assessment, Reviews, CarePlanning and Case Management.
You will also have the opportunity to work across other areas of CHC,including audit, appeals, retrospective assessments, complaints, assessmentverification, and many more.
You will be supported to develop the necessary skills and experience towork across the whole pathway, safely, confidently and effectively.

Job responsibilities

Prior experience of CHC would be beneficial, but is not essential for this role, as you will work within an experienced and supportive team who will support you to develop the relevant knowledge required. However, the post holder should have experience of working within health or care, and will need excellent verbal and written communication skills, as you will be required to work closely with patients, families, carers and other external partners. As a Senior Practitioner, your role will include delivery of a variety of functions across the CHC service, including: - Assessment - Reviews - Care Planning - Case management You will also have the opportunity to work across other areas of CHC, including audit, appeals, retrospective assessments, complaints, assessment verification, and many more. The role will require travel on occasions, to carry out assessments and reviews. Access to transport is therefore necessary, be it personal or public. Skills & Knowledge
* Skills in patient assessment interpreting both physical and mental health needs, skills required to formulate, develop and ensure implementation of specialist CHC care plans to meet identified needs.
* Demonstrates ability to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales
* Knowledge and understanding of current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care
* Ability to address complex or sensitive issues in an organised and appropriate manner both verbally and in writing at all levels; written documents, spread sheets
* Understand and be able to identify and support the Freedom of Information and Subject Access Requests as required
* Knowledge and experience of working with Safeguarding issues and Policy guidance
* Knowledge of the Mental Capacity Act and Best Interests decision making.
* IT literate and working knowledge and computer proficiency of all Microsoft Office packages (e.g. word processing, spread sheets, e -mail and internet use). Intermediate keyboard skills
* Experience of working in multi-disciplinary teams
* An ability to maintain confidentiality and trust
* Used to working in a busy environment
* Demonstrate a desire to improve performance and make a difference by focusing on goals.
* Post holder will need to be able to travel between sites either via personal or public transpor
Qualifications
* Educated to degree level in one of the following fields:
* Allied Health Professions (AHPs)
* Social Work
* With subsequent up to date registration with the relevant regulatory body, including Health & Care Professions Council (HCPC) or Social Work England.
* Evidence of Continued Professional Development
Experience
* Experience of working in a health or care environment (including acute, community, mental health, local authority etc.)
* A good understanding of the health and social care environment and roles and responsibilities within it
* Negotiation, conflict management and influencing skills
* Involvement in operational processes and evidence of using a problem solving and forward thinking approach
