Overview
Shropshire Community Health NHS Trust is looking for a caring, motivated and friendly Community Nurse to join their Community Nursing Team covering the North Shrewsbury locality. The role involves providing nursing care to patients within their own homes or chosen place of residence, offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.
Key Responsibilities
* Provide planned care to patients in the community.
* Deliver nursing care within patients' homes or chosen place of residence.

Working Hours
7 days a week, 8am to 6pm service.
Location
North Shrewsbury locality, Shropshire. The Trust provides services for adults and children in Shropshire, Telford and Wrekin, and some surrounding areas.
