Qualified Social Worker, Hospital TeamPay rate £32.55 per hourContract roleVitalis are recruiting for an experienced Social Worker to work in a hospital team in Westminster Council.Key Responsibilities:
* Triaging referrals on behalf of the Tri-Borough Hospital Discharge Team
* Completing quick assessments for residents of the Tri-Borough
* Undertaking Care Act Assessments and developing Support Plans
* Completing Safeguarding Concern Assessments (Section 42.1)
* Setting up care packages to facilitate safe discharge
* Providing information and signposting to A&E staff
Essential Requirements:
* Social Work England Registration
* Proven experience in hospital discharge work
* Strong knowledge of the Care Act and safeguarding procedures
* Ability to work in a fast-paced environment and manage competing priorities
