This is a rare opportunity for a paramedic to join Bridge Road Surgery to help care for our patients within their home. This includes patients that are in a care home setting.
The role will be a 12 month fixed term contract to cover maternity leave. Within that time the successful candidate will be supported and trained to continue to develop within their role.
The successful candidate will be working with another very experienced prescribing paramedic and an experienced clinical team of GPs and Nurses.
Although experience of general practice would be a benefit, for the right candidate, newly qualified paramedics will be considered as will those that have not worked in primary care previously.
Main duties of the job
* Responsible for attending to patients who wish to be seen by a health care professional within their home (or care home); diagnosing and treating patients presenting with minor illness seeing and advising people in respect of their continuing medical and nursing needs.
* Prioritising and triaging the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
* Work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
Job responsibilities
CLINICAL RESPONSIBILITIES
* Controls admission and discharge to own caseload through:
* Appropriate comprehensive assessment to include physical, mental, emotional and social health including the use of invasive and non-invasive investigations to establish a diagnosis.
* Developing with the patient and family when appropriate, an ongoing plan for health with an emphasis on prevention and self-care.
* Follow-up and evaluation of care.
* Direct referral to and liaison with consultants and other professional agencies.
* Making and accepting referrals within the practice team.
* If qualified to, prescribes according to formulary and competencies.
* Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
* Support and manage health needs of women presenting for family planning, or sexual health consultation.
* Assess, identify and refer patients presenting with mental health needs in accordance with local frameworks.
* A collaborative, supportive and holistic approach.
* To keep accurate patient records on computer system.
Pathological specimens and investigatory procedures
* Undertakes the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs.
* Checks and processes results received via SystmOne as instructed by the GPs.
Administrative Responsibilities
* Disseminates learning and information gained to other team members in order to share good practice and inform others about current and future developments/courses.
* Manages and assess risk within areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
* Participates in the delivery Chronic Disease Management and in achieving QOF Targets.
* Ensures good communication between you and the team and other members of staff and communicate any changes of ways of working to those affected as appropriate.
* Attends and contributes to Practice Clinical Governance.
* Encourages and ensure team working within the practice
* To participate in practice clinical audits if required. Acts as a resource to other practice members.
* Uses technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.
* Reviews and processes data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
* To participate in developing protocols and clinical guidelines for use in the practice.
Qualifications
* Must be a qualified paramedic and registered with the HCPC
