Senior Staff Nurse, Intermediate Care Service (Hospital@Home)

We are redesigning our community services model and implementing a new Intermediate Care Team and therefore an exciting opportunity has arisen to employ nurses to deliver an urgent community response service (provides care 8am to 9pm, 7 days a week). This role will play a crucial part in providing essential support and care to individuals, with a focus on promoting independence, dignity, and well‑being. It is an opportunity for someone who wishes to continue progressing their career in nursing. The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing care or to prevent admission with interventions that can be managed at home. English and/or Welsh speakers are equally welcome to apply.


Responsibilities

* Support the Team Leader in the development of the Intermediate Care (Hospital@Home) Service.
* Facilitate integrated working with local authorities and third‑sector organisations to support individuals within their home.
* Contribute to the development and implementation of policies and procedures within your sphere of responsibility and participate actively in training and education programmes, patient support groups, research, clinical audit, and service evaluation.
* Assist the Team Leader in the day‑to‑day management of the service, utilising effective management and leadership skills.


Welsh Skills Desirable

This post is advertised as Welsh desirable. Speaking and listening skills at Level 3 (equivalent to CEFR B2) or above are considered an advantage. It is not essential but may be taken into account during short‑listing.


Job Details

* Band: Band 6
* Salary: £39,263 to £47,280 per annum pro rata
* Contract: Permanent
* Working pattern: Part‑time
* Location: Trem y Mor, Bettws, Bridgend CF32 8UN


Essential Qualifications and Knowledge

* Registered Nurse.
* Post‑registration qualification to postgraduate Diploma level or equivalent.
* Evidence of post‑registration continuing professional development.
* Extensive knowledge of speciality.
* Working knowledge of multi‑disciplinary teams.
* Working knowledge of clinical governance and its application in practice.
* Knowledge of audit and research.
* Knowledge of safeguarding and adult protection policies and procedures.
* Knowledge of infection prevention and control procedures.
* Evidence of recent continuing professional development.


Desirable Qualifications

* Mentorship qualification or training.
* Clinical leadership qualification and/or training.
* Knowledge of principles of information governance.
* Teaching certificate.
* Local knowledge of statutory and voluntary service.
* IQT – Bronze and Silver Practitioner.


Experience – Essential

* Previous experience of managing staff and resources (team leader or deputy).
* Experience of undertaking clinical audit and/or research in clinical practice.
* Experience in frail elderly and chronic disease management.
* Significant experience as a community registered nurse.
* Experience of implementing innovation and change in clinical practice.
* Experience of inter‑agency working and initiating developments across organisational boundaries.
* Substantial post‑registration experience, most recently in a multi‑disciplinary assessment service for those with complex needs (Intermediate Care or equivalent).


Experience – Desirable

* Experience of working within Primary Care and/or community care settings.
