Job Title: Social Worker – Hospital Discharge Team

An exciting job role has recently become available at the Gloucestershire Health and Care NHS Foundation Trust. They are looking for a new member for their Hospital Discharge Team.
Your Responsibilities:
You will complete comprehensive assessments and provide personalized care that is dynamic and flexible to support service users and their families/carers. You will provide evidence-based interventions and specialist advice, work as a role model within a multidisciplinary team, and enhance the unique value of mental health practitioners. Additionally, you will deliver evidence-based interventions following NICE guidelines to promote recovery, regularly review care and outcomes, support service users to complete clinical outcomes, and measure and monitor progress, adapting treatment options as required.

Qualifications and Experiences:
To be eligible for this role, you must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
