Overview
Our Health in Justice service line is now expanding into non-Custodial healthcare, a person-centred approach delivered via a solid and consolidated infrastructure aimed to assess individuals’ mental health and neurodiversity requirements, to redirect them to the most appropriate channel. We have an exciting opportunity for a Clinical Reconnect Practitioner to join our team in North Yorkshire. As a Clinical Reconnect Practitioner you will work autonomously in police detention cells, in order to identify and respond to requests of screening and assessment of people with identified health needs in contact with the criminal justice system.
Salary - This position is full time, working 37.5 hours per week and you will receive a salary of £42,094 to £47,466 depending on experience.
Candidates are required to have a Legal right to work in the UK due to the nature of the role; offers of employment will be subject to additional vetting and security checks.
Our person-centred model is focused on providing meaningful pathways to anyone faced with criminal charges, as well as understanding what triggers and socio-economic circumstances might lead to people offending. We will treat individuals more holistically, focusing on the person and early intervention strategies to reduce risks and improve outcomes.
Responsibilities
* You will: be part of an extended team working across Liaison & Diversion service
* Work autonomously in prison, community and police detention cells, to identify and respond to requests of screening and assessment of people with identified health needs leaving the prison establishment within a 12-week period
* Use validated tools to assess for a number of identified health needs and utilise information accordingly to inform decision-making and facilitate diversion within and out of the criminal justice pathway, creating a clear individualised care plan

Qualifications, skills and experience
* Qualified Registered Mental Nurse, Mental Health Social Worker or Occupational Therapist with current registration with the relevant professional body
* Knowledge and experience in the female health and social care pathway
* Substantial evidence of continuing professional development and post-registration training
* Experience of psychosocial interventions and awareness of substance misuse
* Excellent communication skills and ability to gain appropriate level of consent
* Understanding of clinical governance, health and safety, QOF, EBP and national standards
* Knowledge of evidence based practice, health promotion, NICE guidelines and pharmacology
