Job overview
An exciting opportunity has arisen for an experienced Clinician to join our dynamic and growing team within Havering’s Adult Mental Health Services. We are looking for a dedicated professional to help build and strengthen collaborative working relationships across inpatient units, support providers, and community services. This role is pivotal in ensuring smooth transitions for patients from inpatient care back into the community, providing the support needed for their continued recovery.
Main duties of the job
Key Responsibilities:
1. Develop and maintain strong communication links between inpatient teams, community services, and external support providers.
2. Act as a key clinician supporting patients through their transition from inpatient care to community settings.
3. Facilitate collaborative working between multidisciplinary teams to ensure holistic care and support for individuals with mental health needs.
4. Work closely with community services to ensure seamless continuation of care post-discharge.
5. Promote patient-centred care while supporting individuals to reintegrate into their communities.
6. Provide clinical input, guidance, and expertise as required, ensuring best practices are followed throughout the transition process.

Starting with Us
NELFT places great importance on new starters being properly welcomed into the Trust. All new starters will join the Trust on the first Monday of each month and undertake a comprehensive induction of up to two weeks, including mandatory training, systems training and the allocation of equipment. As part of the process, new starters will also have the opportunity to meet the executive team, senior managers, and attend several drop-in sessions focusing on engagement, health and wellbeing, and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,313 to a maximum of £5,436 pro rata for part time).
Detailed job description and main responsibilities
This role involves fostering strong working relationships with members of the community MDT, as well as collaborating closely with the inpatient team and ECR colleagues.
A solid understanding of various discharge pathways will be essential to ensure that clients receive personalized care tailored to their needs.
You’ll have the opportunity to shape and expand this role, supporting the reintegration of clients into the community and improving operational processes to create better flow between community services and inpatient care.
Additionally, there is the chance to contribute to the ongoing pilot of the Enhanced Support Pathway, further enhancing the level of care provided
