Meridian Health and Social Care is hiring a Care Navigator based in Preston, Lancashire, Lancashire, North West, UK.
This is a Full time opportunity (£13 per hour).
The service provides an on call support for our domiciliary care branches outside of the usual office hours.
You would be responsible for taking calls from our care teams and service users and providing practical help and advice on various scenario's that occur during early mornings, evenings and weekends.
