North Ipswich PCN is looking to recruit an advanced care practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care. This permanent role is offered on a full or part-time basis.
As an experienced healthcare professional, the ideal candidate will have developed the skills and knowledge to allow them to take on an expanded role and scope of practice caring for patients. The successful candidate will be keen to undertake new ways of working and delivering healthcare, committing to under-taking the professional development required to adapt to changing circumstances.
Main duties of the job
* Demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families and carers.
* Practice autonomously as an advanced clinical practitioner within the designated specialty area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate.
* Support new ways of working that emphasize a more efficient and patient-focused service, ensuring the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area.
* Lead the ongoing development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues.

Person Specification
Qualifications
* Professional registration with relevant professional body: NMC, HCPC or GPhC.
* Post-graduate qualification in specific specialty, or equivalent experience.
* Post-registration experience at a senior level (Band 7 or above) in specialty area.
Experience
* Evidence of ongoing CPD.
* Advanced clinical assessment skills.
* Ability to perform in stressful and complex situations.
* Sound clinical reasoning and decision making.
* Flexibility.
* Ability to inspire and lead a team.
