Nottingham CityCares PublicHealth 0-19 Childrens Nursing Service host the Healthy Lifestyle Team:promoting healthier lifestyles through early intervention and preventionstrategies.

We have been working withfamilies since September 2022 and currently have funding until March 2026 to deliveran enhanced targeted Healthy Lifestyle offer to children aged 0-5 years, andtheir families, who live in Nottingham City.

We are looking for a Nurse whois passionate about promoting health and who would enjoy engaging and workingwith our families. We encourage a think family approach by working together toenjoy healthier lifestyles through buying healthier foods on a budget, cookingand eating healthily, increasing physical activity, better sleep and lessscreen time amongst other changes.

As part of the team, you willhelp to deliver evidence based early intervention and prevention packages ofcare to families and children which will incorporate the above HealthyNutrition, Physical Exercise and Healthier Lifestyle behaviour change.

The service is dedicated toreducing health inequalities by improving the health and wellbeing of childrenand their families, promoting behaviour change with buy in from the families weoffer this service to.

The hours of workwill be 4 days per week. The Service hours are Monday Friday 8.30am 5pmexcluding weekends and bank holidays.


Main duties of the job

The Nurse is integral to thesuccess of the team, leading the triage of referrals to the team, completinginitial assessments of family lifestyles using our assessment tools anddelivering the intervention to families.

You will hold a small caseloadof families who may have more complex medical issues and/or safeguardingconcerns. You will manage the allocation of families to the skill mix team andoffer supervision to the team supporting them with their families.

You will be working withparents and carers of children under 5 years old who have been identified as gainingweight rapidly or as having a BMI that places then at risk of developingcomplications of excessive weight in later life.You work in partnership withfamilies to identify goals that they feel they can work towards to makesustainable changes that will positively impact on their healthy lifestylechoices. These changes can often be small but need to feel manageable and achievableby the family.

The families we work with arefrom very diverse religious, cultural and ethnic backgrounds all of whichimpact on their lifestyle choices. The team continue to develop their knowledgeand understanding of the lived experience of our families and how these impacton the food they eat, how it is cooked and how we can encourage and supporthealthier options.

This role has supported careerdevelopment to enrolment on the Specialist Community Public Health NurseCourse, Health Visiting.

Job responsibilities

Job Purpose

The aim of The Childrens Public Health 0-19 Nursing Service is toreduce health inequalities and improve the health and wellbeing of childrenand their families in Nottingham City. The team offers a universal servicefor all children and young people in Nottingham City as set out in theHealthy Child Programme.

The role of the Community Public Health Nurse is:

To work within a multi-disciplinary team, supporting the SpecialistCommunity Public Health Nurse School Nurse (SN)/Health Visitor (HV) in thedelivery of the universal and targeted services in a variety of communitysettings includingcitywide working and supporting all teams including the ChildrensHealth Advice Hub (CHAH) and Transient Families Team (TFT).

To support the SN/HV inthe management of a defined caseload, as part of an 0-19 years integratedmulti-agency partnership approach.Working to promote health and contribute to tackling healthinequalities in line with local and national guidelines, public health prioritiesand Safeguarding responsibilities.

To work as part of amulti-disciplinary team sometimes without direct supervision but reportingback to the SN/HV on a regular basis.

To work with our most vulnerable childrenwithin Nottingham City with the expectation to attend safeguarding meetingsto include ICPC, RCPC, Core Groups, LACS, ERMM, Strategy Meetings, liaisingwith the caseload holder.

To use a child &family approach to working, in a solution focused way.

To promote holistichealthcare to the babies, children, young people, and families and to work inpartnership with other agencies.

To lead on specificprogrammes of care for babies, children and young people who havespecific/complex needs.

To lead on andparticipate in health promotion initiatives e.g., childhood immunisations, newbornblood spots, in line with local and national guidelines, public healthpriorities and safeguarding responsibilities.

To support thedevelopment of the care pathways required for this population group to reachtheir optimum potential.

To provide advice,signposting and support for parents, carers and young people using TextHealth, our text messaging service.

To ensure that the rights, needs and dignities of babies, children,young people, and families are met by providing health promotion andappropriate programmes of care.

To promote the health ofbabies, children, young people to enable each child to reach his/her fulldevelopment and educational potential which supports the philosophy ofinclusive education.

To work in an inner citywith a rich and varied ethnic, cultural, and socioeconomic climate.

To provide insight opportunities to students visiting the service.

KeyResponsibilities

Clinical

Tofollow a competency-based induction and training pathway supported by yourmentor.

To support the Children and theCommunity Public Health Nursing Team to deliver in Healthy Child Programme 0-19years. This will include development reviews, the National ChildhoodMeasurement Programme.

To undertake home visitingprogrammes to offer support to carers/parents in the care of their childrenfollowing competency-based training supervision.

To work in a variety of settingsincluding clients homes, Childrens Centres, health premises and educationsettings across Nottingham City.

Following allocation ofappropriate work relating to babies, children, young people, and families bythe SN/HV, the post holder will have responsibility for the assessment;development; implementation and evaluation of programmes of care forindividual children and those children with complex health care needs andtheir families

Ability to organise and prioritiseown workload under the supervision of senior members of the team.

You will be able to useappropriate evidenced based tools to assess the physical and emotionalwellbeing of babies, children, young people, and families

To deliver evidence-basedprogrammes of care adhering to standard operating procedures and pathways ofcare

Deliver drop-in sessions andclinics.

To work within CityCare policyand procedure, in line with your NMC code of conduct and NICE Guidance.

Demonstrate good communication,initiative, organisational and interpersonal skills and be able to workflexibly in a team setting & Citywide as the need arises.

Attend team / allocation meetings.

Provide education on local andnational targeted public health initiatives through individual and groupsessions.

Provide and receive routineinformation, orally, in writing or electronically in line with NottinghamCityCare policy and abiding by legal requirements.

Ensure clear, accurate and up todate recording of information on SystmOne as per local record keeping policy.

Maintain an accurate up to date diaryin accordance with lone working policy and adhere to mobile working guidance.

To work in partnership withchildren, young people, and families and with local partners across theNottingham Childrens Partnership, statutory and voluntary agencies

To work with families identifyingand taking appropriate action / make appropriate referrals in any situationwhere there may be vulnerability, safeguarding or health concerns.

To access and participate insafeguarding supervision.

To prepare reports, undersupervision, for child protection, children in need, Looked after Children,Common Assessment Framework and Priority Families. Take the lead professionalrole and attend meetings and reviews where appropriate.

To conduct oneself in a corporateand professional manner always maintaining confidentiality includingcompliance with the CityCare dress code policy.

To attend essential, mandatory,and appropriate training courses and meetings, thus keeping professionaldevelopment current. Ensure practice is evidence based to maintain highquality, effective public health care.

To demonstrate leadership andcontribute to the appraisal, development, and monitoring of performance ofteam members induct and mentor staff and students.

To undertake mentorship,preceptorship, and clinical supervision as per policy.

Professional

Ability to achieve commissionedtargets and understand key performance indicators and service delivery.

To identify hazards and actaccordingly to promote the health and safety of patients, the public andstaff in ls in a health care environment in line with the policies of theorganisation. This includes complying with lone working policies and adheringto infection control guidelines.
