Experienced GP Pharmacist NMP - Harford Health Centre
We are looking for an enthusiastic, motivated experiencedgeneral Practice Pharmacist who is committed to developing their knowledge andskills to improve patient care.
The Practice-based Clinical Pharmacist will improvethe quality and safety of care offered to patients of Harford Health Centre.They will have regular contact with patients and will reduce the workload of GPprescribers in the practice by performing structured medication reviews. Theywill also support the management of patients with long-term conditions byreviewing repeat medication requests.
They will co-ordinate prescribers at the practiceto ensure that appropriate medication is prescribed, ensuring that income anddispensary profit is maintained whilst fulfilling the requirements of CCG andlocality initiatives.
Main duties of the job
The Clinical Pharmacist will developand manage medicines optimisation services within primary care.
The key aims for the clinicalpharmacist are:
To reduce the medicines relatedworkload on doctors
Increase safety and quality ofprescribed medicines
Provide support on medicines relatedissues to the whole practice team
Be a point of contact for patientsregarding medicines queries
A senior advisory role in the day today running of the dispensary
They should support the followingclinical work:
Clinical Medication Review
Chronic Disease Management (forexample, managing hypertension/Type 2 Diabetes/Asthma/COPD patients,polypharmacy review)
Use appropriate CCG provided clinicaltemplates within the EMIS Web Clinical System
Medicines reconciliation on transferbetween care providers
They should also support thefollowing practice development work:
Focusing on QoF Domains, improvingdisease registers
Prescribing Safety, Clinical audit,Local Enhanced Services support
Lead on prescription management(systems and processes, ensuring monitoring is being done, supportingdispensary & clerical (repeat prescription) staff)

Harford Health Centre is located in apurpose built health centre building. This is an exciting opportunity to join aforward thinking modern General Practice. We are a busy practice,dispensing around 11,000 items per month. We have a list size of 10000 patientswhich will be increasing over the coming years as new residences are built inour locality.
Job responsibilities
The Practice-based ClinicalPharmacist will improve the quality and safety of care offered to patients ofHarford Health Centre. They will have regular contact with patients and willreduce the workload of GP prescribers in the practice by performing structuredmedication reviews. They will also support the management of patients withlong-term conditions by reviewing repeat medication requests.
The successful candidate will work as part of a multidisciplinary teamto manage medicines on transfer of care (hospital discharge) and will usesystems for safer prescribing including Eclipse. They will perform face-to-facemedication reviews ofpatients with polypharmacy;especially for those with frailty and/or with multiple co-morbidities. Theywill undertake face-to-face reviews of patients with specific long termconditions that fall within their competency. They will provide leadership on qualityimprovement and clinical audit, as well as managing aspects of medicines safetyand antibiotic stewardship.
They will co-ordinate prescribers atthe practice to ensure that appropriate medication is prescribed, ensuring thatincome and dispensary profit is maintained whilst fulfilling the requirementsof CCG and locality initiatives.
Person Specification
Qualifications
* Masters degree in Pharmacy (MPharm) or equivalent
* Registered with GPhC
* Member of RPS
* Postgraduate knowledge through a Postgraduate Diploma and qualifications (e.g. clinical, community, therapeutics) or equivalent.
* Independent prescriber status
Experience
* Additional qualification in chronic disease or minor ailments would be advantageous.
* Experience in medication reviews and basic clinical skills.
* A community or CCG background who have experience with drug budgets and switches.
