Overview

To support the development of enhanced clinical skill in end-of-life care, ensuring access to appropriate development to maintain a competent workforce. The post holder will provide mentorship and work-based learning facilitation, in-house curriculum design and teaching in end-of-life care. The post holder will perform an expert clinical advisor and leadership role and professional support for the community teams, to ensure that safe and effective, high-quality evidence-based care is delivered in an efficient and cost-effective manner, ensuring that performance management targets are met and evidenced through data production and analysis. Developing clinical governance through the support, monitoring and implementation of enhanced clinical standards and learning within the clinical services. The post holder is an autonomous practitioner and will assess, plan, implement and evaluate patient care and also participate in the training of patients, relatives, carers, students and other health care professionals.

You will be committed to the development of the team through effective leadership and be given the opportunity to develop your own clinical skills through the delivery of a wide range of evidence-based care and an active personal development plan. You must possess excellent communication, assessment and care planning skills and have an excellent knowledge of clinical standards.

Essential requirements include proven Senior Nurse clinical experience at Band 6 or equivalent in an acute or community setting (minimum two years at this level), community nursing experience, experience in end-of-life care/equivalent qualifications, experience of collaboration across organisational boundaries, experience of change management, teaching/mentoring experience, experience of clinical supervision, and ability to undertake audits.

Clinical

* Act as a role model for clinical staff to address patient needs and ensure delivery of safe patient care.
* Keep accurate, contemporaneous records and written reports as required.
* Report and support training and development issues from clinical and non-clinical incidents.
* Proactively manage risk to vulnerable persons and support staff training as identified through development plans.
* Participate / facilitate meetings to address the clinical development needs of staff.
* Work collaboratively with clinical colleagues to identify professional practice development needs for staff.
* Manage risk and maintain service delivery through effective communication with managers, staff and colleagues.
* Engage with clinicians and students to support and enhance quality standards of clinical practice.
* Provide clinical leadership, development and advice based on current evidence in end-of-life care.
* Participate in clinical supervision of registered professionals as required by policy.
* Maintain up-to-date clinical skills and knowledge in end-of-life care and review relevant research.
* Demonstrate personal accountability in everyday practice and oversee staff actions when delegating.
* Contribute to improving patient care and safety by disseminating and integrating evidence-based guidance for physical health, mental health and learning disabilities.
* Incorporate patient preferences into treatment decisions, including recording advanced directives where appropriate (RiO, LCR).
* Ensure the Oxleas NHS Foundation Trust Core Values are embedded.


Education

* Act as a clinical and educational expert in end-of-life care, leading in training and programmes for the Long-Term Conditions sub-directorate.
* Identify learning needs for Clinical Professional staff and develop education programmes around end-of-life care.
* Identify practice areas requiring evaluation through team development planning and training needs analysis.
* Balance practice and education to support different levels of learning and development.
* Evaluate education effectiveness and its impact on service delivery and patient care through audits and benchmarking.
* Facilitate learning by integrating theory to practice development.
* Create a positive learning environment that supports lifelong learning and work-based development.
* Provide support for learners, mentors and specialist practice students (community nursing pathway) in education and development.
* Promote understanding of national and local policies in end-of-life care.
* Develop an end-of-life skills database and career/professional development pathways for the service.


Clinical skills Health and Well Being

* Maintain clinical skills to practice safely and ensure equipment use aligns with Trust standards.
* Exercise professional autonomy and sound judgement to make autonomous decisions.
* Deliver high standard clinical and therapeutic care to patients as per senior practitioner competencies.


Service Improvement

* Work with the Head of Nursing to support professional recommendations for service redesign and new ways of working.
* Act as a change agent to motivate and reassure staff through negotiation and training.
* Ensure effective and efficient use of resources, equipment and supplies.


Quality/Competence

* Practice at your level of competency and maintain professional registration per standards.
* Engage in ongoing professional development and participate in Trust appraisal processes.
* Facilitate data collection.


On Call/Unsocial Hours

The service is currently provided Monday-Friday, excluding bank holidays, 9.00-17.00.
