Social Prescribing Link Worker
Hours of work: 37.5 hours (part time hours considered for the right candidates)
Annual Leave: 30 days plus bank holidays
Salary: up to £27,140 per annum

Responsible to: Social Prescribing Team Manager
Based: Turkey Mill, Maidstone. Working throughout West Kent and able to work from home on agreement with Line Manager.

We are looking for a Social Prescribing Link Worker to work in the West Kent area to provide personalised support for clients to improve their wellbeing. Taking a holistic approach, you will support and connect people to community groups and services to provide them with practical and emotional support.
You will need a positive 'can-do' attitude, high levels of resilience, excellent communication skills and the ability to motivate, empower, and support people to achieve their goals.
Please note, you must be able to drive and have access to a vehicle for this role as it includes community visits and some home visits. This role also includes providing regulated activity with adults therefore employment is subject to relevant checks with the Disclosure and Barring service.
