What you will do:
* Conduct comprehensive assessments of service users’ mental health needs
* Develop and implement personalised care plans to support individuals in the community
* Work collaboratively with multidisciplinary teams to coordinate care and interventions
* Provide crisis intervention and duty service as required, offering immediate support and advice
* Maintain accurate records and reports, ensuring clarity and compliance with regulations
* Engage with service users’ families and other support networks to enhance holistic care
* Facilitate smooth transitions for individuals being discharged from hospital settings
Requirements:
Applicants must be at least two years post-qualified with relevant experience in community mental health or hospital discharge. Working experience on duty is advantageous. A temporary initial contract of three months, subject to review, is offered. Candidates should prepare to start promptly. Our team seeks three social workers for the South Community Mental Health Team and two for the North Community Mental Health Team.
