Primary Care Network Senior Clinical Pharmacist
The post holder will be employed by Greenwich Health the GP Federation and work with Riverview Health PCN, a grouping of 5 practices.
They will work as part of a multi‑disciplinary team in a patient‑facing role, proactively manage patients with co‑morbidities, and provide support to general practice staff with regards to prescription and medication queries.
- Support the repeat prescriptions system, deal with acute prescription requests and medicines reconciliation on transfer of care.
- Provide expertise in clinical medicines advice addressing public and social care needs.
- Provide leadership on medicines optimisation and quality improvement, managing aspects of the quality and outcomes framework and enhanced services.
Main duties of the job
The post holder is preferably an independent prescriber (not essential) and has experience working in primary care. Key responsibilities include:
- Hold patient‑facing reviews and provide medicine support.
- Manage medicines in discharges from hospital.
- Hold long‑term conditions clinics.
- Act as a centre of medicines information for practice staff.
- Support and adhere to repeat prescribing processes.
- Support medicines safety and local/national guidelines.

Pay range: £44,000-£51,000 (equivalent to Agenda for Change Band 8a)

Patient Facing Clinical Medication Review
- Undertake reviews with patients with multimorbidity and polypharmacy, implement prescribing changes as independent prescriber and order relevant monitoring tests.
- Provide patient‑facing clinics for medication queries.
Management of Medicines at Discharge from Hospital
- Reconcile medicines following discharge, manage changes without referral to a GP.
- Produce post‑discharge medicines care plan including dose titration and follow‑up tests.
- Work with community pharmacists to ensure patients receive medicines post‑discharge.
Telephone Help Line
- Provide telephone helpline for patients’ medication queries and follow‑up monitoring.
Medicine Information to Practice Staff and Patients
- Answer medicine‑related enquiries, suggest solutions and provide follow‑up of changes.
Unplanned Hospital Admissions
- Devise and implement practice searches to identify patients at risk of unplanned admissions.
Telephone Triage
- Ensure appropriate referral to healthcare professionals within appropriate time.
Repeat Prescribing
- Produce and implement repeat prescribing policy.
- Manage reauthorisation process, review patient requests and ensure bookings for monitoring tests.
Information Management
- Analyse and present medicines data to support decision making.
- Identify national/local policy updates affecting patient safety.
- Implement guidance for practitioners.
Person Specification
Qualifications
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
Experience
- Minimum 5 years post‑qualification experience.
- In‑depth therapeutic and clinical knowledge and evidence‑based healthcare understanding.
- Appreciation of the nature of GPs and general practices.
Professional Registration
- Registration with the General Pharmaceutical Council.
- Membership of the Royal Pharmaceutical Society.
- Membership or working towards Faculty membership of the Royal Pharmaceutical Society.
