Join our team as a Pulmonary Rehabilitation Team in CountyDurham and Darlington.
We are seeking a dedicated experienced physiotherapist toprovide exceptional care and support to individuals with long-term lungconditions. As a Pulmonary Rehabilitation Physiotherapist, you will play acrucial role in delivering comprehensive clinical triage, assessments,individualised care for patients referred to our service. Your expertise willguide the determination of clinical baseline measures and assessment ofclinical discharge outcomes, ensuring personalised care plans tailored to eachpatient's needs.
Pulmonary Rehabilitation services sits withinthe community care group of CDDFT and isfocused solely on enabling people to live independent and fulfilling lives foras long as possible.
Main duties of the job
Under the superviusion of the band 7 clinmical lead (PR) youwill.
Assess, design, implement, and evaluate personalisedpulmonary rehabilitation exercise and education programs based onevidence-based practices and patient-centered goals.
Monitor and assess patient progress towards goalachievement, adapting exercise plans as necessary in collaboration with thepulmonary rehabilitation multidisciplinary team.
Efficiently and safely triage referrals to the pulmonaryrehabilitation service, ensuring timely access to care while meeting keyperformance indicators.
Manage waiting lists and facilitate appropriate referrals toother services as needed.
Provide specialised guidance and education about thepulmonary rehabilitation service to healthcare professionals across varioussectors, fostering effective collaboration and seamless patient carecoordination.
Produce comprehensive and concise reports forgeneral practitioners and other referring professionals, detailing assessmentfindings and discharge outcomes post-completion of the Pulmonary Rehabilitationprogram.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

To participate in the staff appraisal scheme and PersonalDevelopment Plan (PDP) as both appraiser and appraisee.
To undertake the measurement and evaluation of your work andcurrent practices through the use of Evidence Based Practice projects, audit,research and outcome measures, either individually or with colleagues or withclinical specialist or manager. To makerecommendations for change in service provision.
To liaise with senior colleagues in relation to pulmonaryrehabilitation services within the speciality area and to take an advisory rolein the development of service within the speciality area.
To disseminate guidelines to staff across the Trust in orderto influence and improve standards of care.
To observe and adhere to the Code of Practiceand Rules of Professional Conduct laid down by the HCPC.

Qualifications
* BSc Physiotherapy or equivalent
* Continuing Professional Development Diary including evidence of reflective practice, training and clinical experience
* Experience at B5 level in relevant clinical areas
* Sound knowledge of national and regional policies influencing Respiratory disease
* Recognised relevant academic qualification in relation to respiratory conditions
* An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements and NHS standing
* Accredited COPD and Asthma or respiratory course
* Physical assessment skills course
* Independent prescribing course and prescribing experience
* Accredited teaching qualification (or working towards)
* BTS fundamentals of PR
* CBT basic skills course
Experience
* Comprehensive experience including core competencies within relevant clinical areas (pulmonary rehabilitation &/or respiratory)
* Significant post registration experience some of which must be in respiratory.
* Ability to work within a multidisciplinary team
* Evidence of communication skills within a wide variety of settings
* Experience of prescribing exercise based on individual assessment
* Previous NHS Experience
* Comprehensive experience including core competencies within relevant clinical areas (pulmonary rehabilitation &/or respiratory)
* Significant post registration experience some of which must be in respiratory.
* Ability to work within a multidisciplinary team
* Evidence of communication skills within a wide variety of settings
* Evidence of working in an out patient clinic setting
