Job summary
We have a permanent vacancy for a Practice Nurse to deliver high-quality nursing services to our patient population. The role involves working as part of the practice multidisciplinary team, providing care within an agreed scope of practice.
The Practice Nurse will take responsibility for a range of clinical areas including infection prevention and control, health promotion and chronic disease management, and will support the management team in the review and development of clinical policies and procedures.

We are a forward-thinking, friendly GP practice located in a village setting close to Woodbridge. Our purpose-built premises with allocated staff car parking, sits in the centre of the village, serving a community of just over 10,000 patients, along with a branch surgery at Rendlesham.

Job responsibilities
To provide nursing services to the practice population including chronic disease treatment, screening and advice within the professional competence of the post holder. The post holder will work at all times to the Code of Conduct, as set out by the Nursing and Midwifery Council.
Main duties include but are not limited to:
· Administration of injections
· Childhood & travel immunisations
· Dressings and minor injuries
· Chronic disease management
· Cervical Screening
· Undertake tasks requested by GPs/Nurses and Paramedics such as ECG recording, Phlebotomy, Urinalysis, BP monitoring, NHS health checks, where deemed competent. (Phlebotomy is usually carried out by the HCAs/phlebotomists but, depending on demand or sickness, there will be a need to see patients for venepuncture)
· Ensure all equipment and clinical rooms are clean, safe, and well-stocked.
· Preparing and maintaining environments and equipment before, during and after patient care interventions, including assisting GPs during the performance of minor operations and Coil insertions
· Ordering of Clinical stock
· Helping to raise awareness of health and well-being and how it can be promoted
· Assisting with the collection and collation of data on needs related to health and well-being
· Maintain accurate and up-to-date patient records using clinical systems.
· Assist with health promotion and patient education activities.
· Follow infection control protocols and maintain high standards of hygiene.
· Provide telephone advice for patients and refer as appropriate to other members of the team
· Participate in mandatory training and continuous professional development.
· Working as part of a team whilst also using own initiative, seeing patients independently and efficient time management.
· Aim to achieve business objectives, uphold business values and attain personal goals.
· Good understanding of System One including accurate input of information in a timely manner.
· Utilise the recall system ensuring monitoring and annual reviews are actioned within appointments.
· Develop a thorough knowledge of all practice procedures and work in accordance of protocols
· Providing cover for annual leave/sickness for the health care assistant team.
· Any other duties deemed appropriate by the Operations Managers

Essential -Experience of working in accordance with the NMC Code of Professional Conduct, Experience of dealing with the public and patients, Experience of multi-skilled approach to Chronic Disease Management, Demonstrate performance on the Quality & Outcomes Framework

Experience of working in general practice
