An exciting opportunity has arisen to come and join our Respite care team supporting young people with a learning disability Children and adults with complex health care needs. You will work as part of team, and liaise with external collogues from social care to community learning disability staff, families and carers. The Successful candidates will be able to demonstrate great communication skills alongside a commitment to developing a positive team culture focused on person centered care. This role provides an opportunity to develop skills and experience working alongside a dedicated team of professional staff, while also developing your own skills. The service will provide a range of training opportunities aimed at supporting your continuing development.
These roles will also provide support to the Senior Support Workers and will interlink with the Unit Manager, Team leader, deputy team leader and Administrator and other professionals working within the service.
You will routinely interact with service users, their families and carer's and also a wide range of professionals from across Bury area
Main duties of the job
The post holder will predominantly work within Cambeck close respite care service but may be asked to support other areas with the learning disability care hub such as community teams and Crisis Response Unit in Radcliffe or within the footprint of Pennine Care.
The post holder will provide high quality care and support to people accessing the service. They will assist in the delivery of the model of care under the guidance of the Team Manager, deputy team manager and Senior Support Workers,
People accessing the service may be experiencing periods of distress and at times present as challenging or require support with complex health conditions such as Peg feeds, oxygen, epilepsy and diabetes. The post holder will provide support which is therapeutic while also maintaining the persons safety and dignity.
The post holder will assist in establishing care plans and in the delivery of care. Reviewing care plans, risk assessments and triaging referrals into the services. The candidate will support junior staff with line management supervision and IPDRs, or any other HR related matters. The expectation will be to work within the boundaries of agreed instruction, whilst being supported within Trust procedures and policies

Detailed job description and main responsibilities
* At all times to work in accordance with Pennine Care policies and protocols.
* To be responsible for the assessment of care needs, for an agreed group of service users.
* The development, implementation and evaluation of programmes
of support, and contribute to the setting of a high standard of care to clients,
* embracing the values of ‘Valuing People
* To fully involve clients, family members and persons acting as advocates in the development of the above care plans and risk assessment.
* To adopt a multidisciplinary approach to the development of programmes of care.
* To take on the role of Named Nurse for a number of identified clients.
* To maintain accurate and up to date records in line with Pennine Care and NMC guidelines.
* To transcribe and administer medication ensuring that legal requirements are observed in line with Pennine Care policy and procedures and NMC guidelines.
* To communicate effectively with a range of agencies including GPs, CTLD, Social Services, etc.
* To operate as a leading member of the staff team, making a prominent contribution to Team Meetings, Staff Development and training and overall development of the Pennine Care Learning Disability Service.
* Provide day to day formal and informal supervision for members of the support
team.
* Receive formal and informal supervision from the Service Manager and Identified Team Leader.
* To contribute to the professional development of the support workers including NVQ/apprentership courses.
* To complete appraisals for identified support team members.

* To take a lead role in the development, implementation and monitoring of risk assessments for service users.
* To be responsible for the deployment of Support Staff also having responsibility for the service in the Managers and Team Leaders absence.
* The post holder will be involved in at least one service development group across Learning Disability Partnership.

* •Experience of working with Children and adults
* •Experience of working with people with a learning disability and/or autism
* •Experience of providing care and support in a community setting and/or residential setting

* •Understanding of positive behavioral support
* •Able to impart knowledge, supervise and direct the work of others under supervision

* •Able to remain calm in stressful situations and display patience and empathy
* •Able to develop and maintain effective therapeutic relationships patients and families
* Be able to provide supervision to junior staff
