This role offers an exciting opportunity within Urgent Care for a Lead Case Manager for the High Impact Service. The successful candidate will be an experienced registered nurse, responsible for managing the Frequent Attenders Service and leading clinical and administrative teams. The role involves strategic development, operational management, and providing expert advice to support complex and vulnerable patients, in line with the Six Goals for Urgent and Emergency Care.

The High Impact Service was established in 2019 to support patients who frequently attend emergency and urgent care services, addressing social, health, and systemic factors contributing to their repeated presentations. The service now also supports hospital discharge management and multi-agency collaboration to reduce unnecessary attendances and improve patient outcomes.

The main aims of the service include ED avoidance, reduced length of stay, reducing unnecessary investigations and admissions, improving patient experience, safeguarding, staff protection, education, and addressing health inequities through partnership working.
