Bowel Screening Accredited Clinical Endoscopist

The Band 8b Bowel Cancer Screening Clinical Endoscopist is a senior autonomous practitioner who provides expert diagnostic and therapeutic endoscopy within the NHS Bowel Cancer Screening Programme (BCSP). The post holder works at an advanced and strategic level, delivering high-quality, safe, and effective endoscopic care, while also providing clinical leadership, service development, education, and governance across the bowel cancer screening pathway.

The postholder will be skilled in both diagnostic and some therapeutic colonoscopy and will be expected to work across all screening sites with Calderdale, Kirklees and Wakefield. They will use expertise to maintain and improve the quality and consistency of health care experienced by patients and service users within both BCSP and symptomatic endoscopy services.


Main duties of the job

* Performing independent, high volume colonoscopy within both the BCSP and the symptomatic service, including endoscopic therapy appropriate to the candidate’s limitations.
* Maintaining JAG and BCSP performance standards, ensuring maintenance of acceptable Key Performance Indicators (e.g. caecal intubation rate, SPA detection rate).
* Providing expert clinical decision making across screening and symptomatic pathways, including escalating care for the unwell patient, arranging further investigations and determining appropriate surveillance pathways.
* Acting as a clinical expert and point of reference when complex cases and adverse events arise.
* Working autonomously whilst performing screening and symptomatic colonoscopy on participants requiring IV conscious sedation. The post holder will occasionally work alongside an anaesthetist in situations requiring deep sedation or a general anaesthetic.
* Appropriately maintain and update clinical knowledge and skills and use this to effect change appropriate to the ongoing development and expansion of the National Bowel Cancer Screening Programme.
* Acting as a lead within the clinical endoscopist team, overseeing and supporting the team’s development and compliance in all aspects of their roles.

Job responsibilities

Hands‑on skill and coordination are required to perform diagnostic and therapeutic screening and symptomatic colonoscopy, prioritising patient safety and comfort. The postholder must maintain physical effort during examinations and sustain concentration, using digital and chemical enhancements to identify lesions. Appropriate PPE must be worn due to exposure to bodily fluids. Physical demands include standing for long periods and exposure to noise and heat, with high‑definition video display units used throughout procedures. Infection control measures must be consistently applied as per national guidance and local policy.

The postholder will communicate effectively with various individuals from within the screening and symptomatic MDT, other professional groups, organisations, patients and their carers, and will convey complex interventions, risks and outcomes in a clear manner, respecting equality and diversity. The postholder must understand and apply the legal safeguarding framework that supports the identification of vulnerable adults.

Delivering bad news, ensuring further staging investigations are requested and appropriate MDT referrals are made. The postholder will provide ongoing support, including named contacts, to address enquiries, questions and concerns. In‑depth knowledge of lower gastrointestinal diseases and disorders is required. Endoscopic findings must be interpreted to distinguish benign from potential or actual malignant disease, initiating appropriate actions.

Practice within own limitations and escalation of care where appropriate to ensure patient safety. The postholder may be an independent non‑medical prescriber or willing to undertake such training. The postholder will administer IV conscious sedation, including reversal agents and resuscitation equipment, and manage post‑polypectomy bleeding. Large or complex benign polyps will be referred to a site‑specific Complex Polyp MDT with photo documentation and SMSA criteria.

Non‑malignant disease (e.g. IBD) detected at screening colonoscopy must be referred appropriately, with urgent findings alerting the relevant medical/surgical team. Recognition and management of complications attributed to colonoscopy is essential, with decisive action required.


Person Specification


Qualifications and Training

* Registered Nurse
* Independent non‑medical prescriber is preferred but trainees may apply.
* Completion of the HEE Clinical Endoscopist Programme or equivalent.
* Training the Trainer (JAG) qualification.
* Teaching qualification.


Knowledge, Experience and Expertise

* Up‑to‑date professional knowledge and future provision of the service.
* Knowledge of Endoscopy service and JAG accreditation.
* Evidence of effective leadership and developmental work.
* Evidence and facilitation of change management.
* Comprehensive nursing knowledge, skills and experience.
* Exceeding acceptable Bowel cancer screening KPI’s.
