Job overview

We have an exciting opportunity for a Band 3 Mental Health support worker to join this dynamic team. Oldham Older Peoples Intensive Home Treatment team on a 6 month fixed contract. The post holders will work Monday to Fridays as part of the team to deliver an effective and efficient response to people who access the service.

Oldham Older peoples Intensive Home Treatment Team (IHTS) is 7 day a week service working with males and females over 65 with mental health conditions in an acute stage of their illness. IHTS works as an alternative to a hospital admission in an attempt to provide intensive support to patients within the community and prevent hospital admissions.

We are also able to screen inpatients with a view of supporting and facilitating early discharge from an acute inpatient setting.

We work alongside patients and their regular care providers (CMHT, Care Homes etc)

The IHTS team is made up of experienced RMN nurses and support workers, working alongside a Consultant Psychiatrist


Main duties of the job

The post holders will work as part of the Oldham Older People Home Treatment Team, to undertake allocated duties within the team, you will work alongside mental health practitioners, supporting patients, families and carer's in their own home and also supporting patients who are on the supported discharge pathway from the wards ensuring the highest possible standard of care in accordance with the policies and procedures of Pennine Care NHS Foundation Trust.

The postholder will work five day a week Monday to Friday, 9a.m. until 5p.m.

Detailed job description and main responsibilities

* To provide support to an allocated group of service users, promote their recovery and help to maintain them in their community environment.
* To assist qualified practitioners in assessment, planning, implementing and evaluating individual care plans.
* To ensure that service users engage effectively with the agreed Care Plan and access appropriate services.
* Positively promote independent living of service users within the community
* Provide support and interventions to service users in their own homes and the wider community based upon their assessed needs.
* Assist service users to make full use of community resources.
* Support service users to become involved in social activities in line with their assessed needs and planned care.
* Support service users to establish and maintain effective interpersonal relationships.
* Provide regular and practical support for service users and their carers’ in managing dignity and independence.
* Provide support with activities of daily living.
* Help service users gain access to resources required to meet their needs.
* Provide information on health promotion and support physical health management.
* Help to identify early signs of relapse by monitoring the service users progress, level of functioning and mental state and to alert the appropriate staff involved in the service users care.
* Report regularly to other members of the multidisciplinary team
* To promote independence and choice.
* To maintain adequate records as required by existing procedures, entering appropriate details on the service users case notes as necessary.
* To attend and actively participate in training sessions, team/care planning review meetings and supervision as appropriate.
* To contribute to the ongoing development of the service
* To ensure that electronic care records are efficiently maintained.
* To contribute to audits as required, providing relevant data as required in line with performance monitoring targets.
* To deal appropriately with clinical incidents and complaints from patients, relatives and members of the public, via line manager, in accordance with Trust policy.
* To undertake such other duties as may be determined from time to time within the general scope of the post.
* To assist qualified practitioners in undertaking physical health checks and monitoring in line with the new community clozapine titration pathway.

Education / Qualifications


Essential criteria

* •Good standard of general education including good numeracy and literacy skills.
* •NVQ 3 or equivalent
* •Commitment to undertake Continuing Personal Development

* •Evidence of recent further development / training.


Experience


Essential criteria

* •Experience of working with individuals with mental health needs
* •Experience of working with individuals in distress


Desirable criteria

* •Experience of lone working


Knowledge


Essential criteria

* •Knowledge of physical health observations.
* •Understanding of mental health issues and how these impact on individuals.
* •Understanding of confidentiality and data protection issues.

* •Awareness of local services.
* •Knowledge of benefits / employment systems.


Skills and Abilities


Essential criteria

* •Good written and verbal communication skills
* •Ability to work unsupervised in a range of settings.
* •Ability to provide practical support with daily living activities.
* •Ability to form positive therapeutic relationships with service users and carers.


Desirable criteria

* •Willingness to engage with or have undertaken Venepuncture and physical health checks training

* •Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
* •Able to pass Occupational Health Screening process
