We are looking for an experienced practitioner to deliver trauma-informed, evidence-based support to adults affected by substance misuse. This role involves managing a complex caseload, conducting specialist assessments, care planning, and delivering psychosocial and clinical interventions. You’ll work closely with multi-agency partners to improve clients’ health, resilience, and social outcomes.
Key Responsibilities:

Carry out comprehensive assessments and deliver individual and group interventions focused on substance misuse recovery.

Support clinical activities including prescribing, drug screening, and physical health monitoring.

Develop and review personalised care plans to address wider health and social needs.
*
Manage risk effectively and ensure safeguarding standards are upheld.

Collaborate with partner agencies to ensure coordinated, holistic care.
*
Lead and participate in community outreach and targeted awareness campaigns.
Candidate Profile:

Proven experience in adult substance misuse services.
*
Strong communication and relationship-building skills to engage vulnerable adults.
*
Knowledge of trauma-informed, evidence-based intervention techniques.
*
Ability to work flexibly to meet client needs, including occasional evening work.
