Early Help Lead Practitioner, Wakefield
Pay rate to £18.35 per hour

Vitalis are recruiting for an experienced Early Help Lead Practitioner to work in a team in Wakefield.
1. To work as part of a multi-disciplinary team to support the delivery of the Early Help Offer for families, adopting a whole family, strengths-based approach in order to reduce risk, increase resilience and improve outcomes for children.
2. The post holder will have core functions as a lead practitioner for Targeted Early Help, working with other agencies and directly with children and families to improve identified outcomes.
3. Case management of packages of support for families with multiple needs, undertaking assessments, developing plans of support, and taking a lead co-ordinating role, including co-ordination of intensive multi- agency support packages where needed and leading on multi-agency reviews of the support plans.
4. Rapid response to Early Help referrals received into the Integrated Front Door to determine the immediate needs of a child, young person or family and to support information gathering to enable appropriate allocation.
5. To report on outcomes as part of the Children and Young People’s Performance Framework. To ensure that services are delivered in compliance with relevant legislation, local and national policy and guidance.
