£33,552 per annum; 36 hours per week; hybrid working with a minimum of two days in the office and community visits.

About the Role
As a Senior Social Care Assistant you will often be the first point of contact between the public and the service, gathering initial information, determining the need for further assessment and sign‑posting people to other services.
You will hold a caseload and work with individuals who need additional support or guidance to identify their specific needs or where a best‑interest decision must be made on their behalf. Interventions may be required to minimise and manage risk, and you may be involved in risk assessments, supporting safeguarding enquiries and attending safeguarding meetings.
You will collaborate with young people, families, SEND schools and other professionals to create an individualised plan from Year 9 through to adulthood, supporting them until they turn 25.
You may work with any combination of care groups: people with physical, sensory, cognitive impairments, autistic people, people with a learning disability and people with mental‑health problems. The role includes visiting people in their own homes, identifying and arranging services to promote independence, choice and control.
Qualifications
* Strong empathy and compassion towards individuals in need of social care assistance.
* Substantial experience working with residents with a variety of physical and cognitive social care needs, including strength‑based assessments, home visits and support plans.
* Knowledge of the Care Act legislation and its impact on work.
* Ability to take initiative and adapt to shifting priorities in a dynamic environment.
* Excellent communication skills, both verbal and written, with the ability to convey information clearly, concisely and respectfully.
* Competent IT skills and willingness to learn new software systems.
Additional Requirements
Full DBS disclosure and Barring Service checks (including the Children’s and Adults’ Barred List) are required.
A valid UK driving licence, access to a vehicle and willingness to travel across a wide geographical area are necessary.
