Pembrokeshire Intermediate Care Team are offering an exciting opportunity for a motivated, enthusiastic and passionate Band 3 Nursing Support Worker to join our Care at Home team .
The successful candidate will be competent to work without direct supervision, within a framework that has been agreed, to provide high quality care to patients including end of life, therefore reducing the need for acute hospital admission and facilitating earlier discharges.
As part of the Care at Home Team y ou w ill ensure that patients receive
safe, individualised and effective standards of care by following care plans
and treatment interventions as directed by the designated Coordinator .
The successful candidate will need to communic ate effectively with other
team members at all times across all of the integrated teams and
demonstrate a flexible approach to the needs of the service.
Community experience is desirable, but not essential as induction and
training will be provided.
The ability to travel in a timely manner across the Health Board is essential .
The ability to speak Welsh is desirable for this post; English and/or Welsh
speakers are equally welcome to apply .
