An exciting opportunity has come up for the Community Nursing Service. We are currently looking for Band 7 District Nurse to work across the Hounslow boroughs.

Collaboratively working with our integrated rapid response teams, community matrons and social service colleagues are essential to ensure seamless care to our patient group.

The candidate needs to be very committed to the role, lead by example and be a role model to the team. Have an open-door policy for all staff, be very approachable, a good problem solver and a good listener.

Communication is vital to this role; the candidate should be able to liaise with other MDTs using professional language and have the ability to manage challenging situations and complaints.

Main duties of the job

To provide high quality, skilled nursing care and to assist the Community Nursing Team Manager in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.

Deliver high quality nursing care to housebound patients with complex needs to enable them to remain in their own homes. Working closely with specialist nurses, allied health and social care professionals to meet the needs of the patients without unnecessary attendance in A&E or be admitted to hospital.

This will involve working independently but as part of the Community Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care. Candidate will be expected to be flexible to support corporate approach to patient care as and when required.

Essential criteria

1. Registered Nurse Level 1 with current NMC Registration. Recognised nursing qualification at degree level. SPQ DN Qualification

2. Significant clinical experience in bladder and bowel health at a senior level.
3. Experience of clinical leadership and supporting others to delivery high quality care

Desirable criteria

4. Significant experience of working in the community, primary care setting or acute.
5. Experience of working in a multi-disciplinary team.
6. Experience of managing and supervising clinical staff.

Skills

Essential criteria

7. Ability to work independently and as part of a multi-disciplinary team.
8. Ability to manage complex clinical cases.
9. Excellent communication skills, written and oral
10. Willingness to work and travel within geographical area of service. A car driver or has access to own transport.
