Unplanned Care Community Staff Nurse (Band 5)
Join our community nursing team to provide high-quality care in patients’ homes. The Unplanned Care Team responds quickly to assess and manage presenting issues, prevent unnecessary hospital admissions, and facilitate early discharges.
The post holder will work with a multidisciplinary team to achieve the best outcomes for patients, operating on a seven-day, 24-hour rota with agile technology to support timely, high-quality care.
Responsibilities
* Deliver high standards of care in patients’ homes and participate in the assessment of health needs.
* Provide care to the practice population with a focus on preventing hospital admissions and facilitating early discharges.
* Work collaboratively with the wider Community Partnerships and Primary Care Networks.
* Manage your own workload and contribute to service development and multi-agency working.
* Demonstrate patient-centered care and effective communication with patients and the multidisciplinary team.
Qualifications
* Qualified nurse or current nursing student/in training seeking to work in a community setting.
* Experience in community care is an advantage; the post is also suitable for newly qualified nurses.
* Willingness to undertake tailored professional development, including a preceptorship programme.
