Job Overview THIS POST IS FIXED TERM FOR 12 MONTHS (24/10/2026) DUE TO MATERNITY COVER Cardiac Rehabilitation Exercise Professional, Band 6 An exciting opportunity has arisen for a suitably qualified and enthusiastic Exercise Professional/Physiotherapist to join this well established cardiac rehabilitation service. The post holder will join the multidisciplinary team to contribute to the delivery of cardiac rehabilitation care in a variety of settings. The role will involve leading on and implementing cardiac rehabilitation interventions in Wrexham and Flintshire, with requirements to travel across Wrexham and Flintshire. The client caseload includes those following acute coronary events and/or coronary intervention and those with heart failure. The postholder will promote independence, confidence and improved quality of life for both the client and their family where necessary. You will require excellent communication and motivational skills and be able to work effectively both within a team and on your own. Part time - 30hrs per week Flexible between Monday to Friday, between the hours of 8.30 – 16:30pm (days to be confirmed to suit the needs of the service) Base: Wrexham Maelor Hospital or Connah’s Quay Health Centre, with travel across Wrexham and Flintshire. Develop & expand cardiac rehabilitation exercise-based services to meet the needs of individuals and groups with cardiac conditions To act as specialist resource for the cardiac rehabilitation team and National Exercise Referral Scheme (NERS) staff for Supporting the provision of the core components for cardiovascular disease prevention and rehabilitation as outlined in BACPR Standards and Core Components for Cardiovascular Disease Prevention and Rehabilitation 2012. Work as a member of the multidisciplinary team in the management of patients with cardiac conditions, some of whom have highly complex physical and psychological problems, that promotes independence, increased confidence and improved quality of life for patient and family where necessary. To support the development of junior and less experienced staff in relation to exercise in cardiac rehabilitation. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. Detailed Job Description And Main Responsibilities Assess the clinical suitability of patients attending for exercise and prescribe exercise to patients with wide ranging pathophysiology and co-morbidity. To lead and implement individualised cardiac rehabilitation sessions in line with current guidelines within a variety of settings to include patients’ homes, leisure and community centres and hospital outpatient departments. Deliver interventions to groups of people as well as individuals. To ensure the smooth organisation of group sessions, clinics, home visits and other patient (and significant other) contact, taking into account content, group numbers and evaluation of intervention using standardised outcome measures. Person specification Qualifications Essential criteria Degree in physiotherapy with HPC registration OR Degree in sports and exercise science and registered as BASES certified Exercise Practitioner or Accredited Sports and Exercise Scientist OR Degree in exercise physiology and registered as BASES certified Exercise Practitioner or Accredited Sports and Exercise Scientist OR Level 4 Cardiac Disease (Rehab) qualification in exercise fitness e.g. BACPR Exercise Instructor Training qualification and with current REPs at Level 4 AND supplemented by specialist courses/training and experience in the field of cardiology to post-grad diploma level or equivalent Desirable criteria Masters level qualification in relevant field eg cardiac rehabilitation Experience Essential criteria Has worked as a band 5 or equivalent Exercise Professional Evidence of working with complex patient groups Prescribing and leading exercise sessions to Cardiac Patients Desirable criteria Previous NHS experience Service development Skills And Knowledge Essential criteria Ability to assess clinical information and structure patient care accordingly Skilled in delivering educational talks and lead group exercise sessions Risk stratify and prescribe safe and effective exercise programmes Up to date knowledge of national and local strategies to reduce Cardiovascular Disease (CVD) Good knowledge of existing exercise testing and prescription guidelines Knowledge of cardiac conditions and treatment and how this may affect exercise capacity/ability Desirable criteria Ability to teach exercise physiology to non exercise staff personal qualities Essential Criteria Good interpersonal and communication skills Enthusiastic, dynamic and outgoing personality Ability to work on own initiative and to work as part of a multidisciplinary team
