An exciting opportunity has arisen for a Health and Wellbeing Practitioner to join our Community Mental Health Team within Navigo. As part of the team, you will have the opportunity to establish therapeutic and educational groups for our clients. You will be able to formulate care plans, deliver focused psychologically informed interventions on a one-to-one basis or in a group environment. You will be supported and supervised by our psychology services, as well as a senior team lead, and will have peer support from other Mental Health and Wellbeing Practitioners. Main Duties of the Job
This is a specific role within Adult Community Mental Health Services. The post-holder will hold a certificate or postgraduate certificate in Mental Health and Wellbeing Practitioners. The post-holder will work within the limits of their experience and assessed competence. They will work alongside service users, families, carers, and multidisciplinary team members to coordinate care, supporting collaborative decision-making about care and treatment. They will deliver a set of wellbeing-focused psychologically informed interventions, aligned with cognitive-behavioural principles, based on the best evidence available, addressing problems often experienced by people with severe mental health problems.

Job Responsibilities
Carry out relevant risk assessments and risk management with the multidisciplinary team.
Engage calmly, with sensitivity and empathy, to support service users in highly distressing or emotional circumstances. Engage with and gather information from service users, relatives, and patient records to develop a formulation. Work collaboratively with the individual service user and their family and carers, as appropriate, to develop strengths-based, outcome-focused care plans. Attend multidisciplinary reviews and act as a care coordinator for a caseload of service users to support and monitor progress during interventions. Set collaborative goals for intervention with service users. Deliver specified wellbeing-focused psychologically informed interventions, aligned with best available evidence, under close supervision from a clinical psychologist or CBT therapist, including behavioral activation, graded exposure, problem-solving skills, sleep improvement, emotion recognition and management, guided self-help for bulimia and binge-eating, confidence building, and medication support.
Be responsive to service users' needs and choices, uphold their dignity and respect, and include carers and families in line with the service user's wishes.
Liaise with other health and care providers, including third sector agencies and primary care, to ensure continuity of care. Communicate assessment, formulation, and care plan information in a skilled and sensitive manner to promote effective multidisciplinary working and therapeutic outcomes. Develop collaborative relapse prevention plans. Manage endings appropriately and safely with service users, families, and carers.
Communication
Form professional relationships with service users, respecting their views, autonomy, and culture. Monitor and record progress, highlight concerns, and report changes in wellbeing to the team. Demonstrate effective communication skills when discussing sensitive and complex information, ensuring understanding. Provide relevant written and oral information for records and reviews. Communicate regularly and in detail with supervisors and case management teams, utilizing clinical skills and supervision effectively to plan, reflect, and implement best practices.
Documentation
Maintain up-to-date clinical records in accordance with professional and Trust standards.

Professional Development
Complete and stay updated with all statutory and mandatory training. Attend training and meet requirements, including assessments. Apply learning in practice. Receive supervision related to coursework and clinical practice. Prepare and present clinical information for caseload reviews and supervision, responding to feedback and improving practice accordingly.

Qualifications
* You must hold a certificate or postgraduate certificate in Mental Health and Wellbeing Practitioners or be practicing as a Psychological Wellbeing Practitioner.
Experience
* Experience of working with the public.

* Experience analyzing and communicating complex information verbally and in writing.
