Person specification Education/Qualification Essential criteria Registered Nurse (RN) or Paramedic NMC Registration or HCPC Registration Desirable criteria Evidence of other post-registration education and training relevant to post Experience Essential criteria Recent clinical experience in community / domiciliary services. Working with people with complex needs undertaking comprehensive assessment and care planning. Experience of working as part of a multidisciplinary team Changing own practice in response to changes in service Recent clinical nursing experience negotiating and liaising within a multidisciplinary team Working with people with complex health needs Desirable criteria Leadership, support, induction, and teaching of other staff Evidence of participating in investigation/ management of complaints Evidence of participating in clinical audits or service development projects in the workplace Post registration experience in a NHS community environment Skills and Knowledge Essential criteria Knowledge of out of hospital care and the role of the Community Nurse Ability to listen effectively and ability to give constructive feedback Time management skills Knowledge of how Equal Opportunities can be implemented in practice Knowledge of reasonable response to service user needs and prioritising and dealing with those needs with a required urgency.
