JobDescription:
* Thespecific clinical and managerial responsibilities of the postholderwill be as follows:
* To provide advice andsupport to GPs and other practice staff regarding the assessmentand management of the mental health difficulties of patientsregistered with their practice. This may be done through casediscussions joint consultations and bespoke training asappropriate.
* To provide consultant input andclinical leadership to the multidisciplinary team. To assist withdiagnosis formulation medication reviews management plans riskassessments crisis plans and selfhelp plans as required for thesepatients referred to the team.
* As necessary toprovide direct clinical assessments of patients registered with theGP practices and to provide follow up asappropriate.
* To assist in the prioritization(triage) of new referrals to the service.
* Tomaintain effective communication with GPs primary care staffpatients and their carers.
* To work flexiblywithin timetabled working hours so that urgent clinical matters canbe prioritised. To carry out Mental Health Act and domiciliaryassessments as required.
* To play a key role inimplementing improved linkage between primary and secondary careservices.
* To liaise and work closely with localIAPT services (also provided by C&I) and other primary careproviders in order to provide a joined up primary care mentalhealth service.
* To liaise and work closely withlocal voluntary services as part of the new model of core communityteams
* To provide clinical input where necessaryto multidisciplinary case conferencing around difficult patientsbeing managed solely in primary care.
* To engagewith local service user participationmodels.
* To participate in management activitiesand governance processes related to the Practice Based MentalHealth service including audits and quality improvementprojects.
* To play an integral role in theperformance of the team the further development of the service andthe development of clinical pathways and priorities of the serviceline.
