Overview
We operate an 8 am to 10 pm service, seven days a week, in the Upper Lea Valley. We are looking for an enthusiastic and motivated Caseload Management Nurse to provide high-value health care to patients in a community setting under the direction of a Deputy Locality Manager.
Responsibilities
* Lead and supervise a diverse team of Band 7, Band 6, Band 5 and Band 3 nurses to ensure patients receive high-value healthcare.
* Work autonomously and demonstrate excellent communication skills and collaborative working across the integrated community team.
* Understand and address current professional and community issues, staying innovative and responsive to continual developments within community nursing services.
* Commit to flexibility and adaptability to meet evolving community nursing needs.
Qualifications and Requirements
* Proven community nursing experience.
* Excellent communication skills and ability to work collaboratively.
* Commitment and flexibility to meet changing service demands.
* Full valid driving licence and access to a car for business purposes (unless disability under the Equality Act 2010).
