Highly Specialist Speech and Language Therapist
The postholder will be a member of the Adults with Learning Disability Team, The team works across the boroughs of Lambeth, Southwark and Lewisham. The post holder will be working with adults with a Learning Disability who have communication impairments and / or dysphagia.
The post offers excellent opportunities to work jointly with other Speech and Language Therapists who specialise in Adults with Learning Disabilities as well as other members of a multi-disciplinary health team.
The post holder will be one of a team of speech and language therapists who support people who have dysphagia. The post offers opportunities to for the successful candidate to support people and their carers to use a variety of Augmentative and Alternative Communication approaches.
Main duties of the job
To assess, diagnose, design and deliver appropriate speech and language therapy intervention for people with learning disabilities who have communication and or eating and drinking disorders
To provide highly specialist advice to others regarding the management and care of communication and/or eating and drinking difficulties
To develop appropriate strategies for managing and prioritising complex caseloads in community settings independently

Main bases for the Service are in East Dulwich and Catford.
The Post holder will primarily work across multiple sites in Southwark for clinical work, but will be in a multi-disciplinary team and also work across Lambeth and Lewisham
Integrated and Specialist Medicine consists of 9 Directorates and provides care and treatment to patients who require emergency and inpatient care, community services including district nursing and specialist care, dental services, specialist inpatient / outpatient areas, imaging and pharmacy. Integrated and Specialist Medicine also have responsibility for cross site operations. The integrated approach of the clinical group facilitates and promotes the shared learning of best practice, innovative and creative ways of working to enhance the patient experience and ensure the delivery of outstanding care
The Integrated Local Services directorate within this clinical group provides a range of community and inpatient health services for adults within the communities of Lambeth and Southwark, with some services also provided in Lewisham. the Adults with Learning Disability team sits within this directorate.

Job responsibilities
Responsibility for the assessment, diagnosis, development and delivery of highly specialist Speech and Language Therapy communication assessment and interventions to meet the needs of people
Responsibility for the assessment, diagnosis, development and delivery of highly specialist Speech and Language Therapy eating and drinking (dysphagia) assessment and interventions to meet the needs of people
To maintain adequate client case notes, upholding the minimum standards for the service, and to provide written reports as per quality standards
To write highly-specialised reports in multiple formats so that, where possible, they are accessible to the client as well as their carers, and other professionals
To ensure that clients are actively involved in the planning and prioritisation of their own care plan
Responsibility for communicating effectively with staff, support workers, other members of the multi-disciplinary team and wider professional team, GP, family members and all significant others on aspects of client services
To put in place, manage and use information from systems and processes that communicate across all of the service users environments, including their home e.g. participation records, communication monitoring systems, eating and drinking monitoring forms
Person Specification
Qualifications
Essential
* Membership of the Health Professions Council.
* Masters level qualification in Speech and Language Therapy/Science or equivalent experience.
* Postgraduate qualification in dysphagia.
Desirable
* Advanced dysphagia qualification
* Makaton Tutor
* Clinical Audit Training
Knowledge
Essential
* Demonstrated skills in dysphagia assessment and management.
* Demonstrated skills in the use of AAC eg Talking Mats, Makaton ( keyword signing), Light tech switches
* Demonstrated ability to prioritise workload and work on own initiative.
Desirable
* Demonstrated ability to supervise exam placements for students.
* Demonstrated ability to integrate Government legislation into care plans and individual Education, health and Social Care plans, as appropriate
* Demonstrated ability to supervise placements for students.
Experience
Essential
* Experience of training carers, volunteers and health care professionals and education staff.
* Experience with undertaking risk assessments
* Experience of working within a multidisciplinary team.
Desirable
* Experience of supporting AAC-users who are communicating with high-tech communication devices e.g.Ipad eye gaze
* Experience of liaison with health, social services, education and voluntary organisations
* Experience of developing clinical care pathways
Skills
Essential
* Demonstrated skills in the use of AAC eg Talking Mats, Makaton ( keyword signing), Light tech switches
* Demonstrated ability to prioritise and manage risk within the commissioned resources. To advise SLT manager of any unmet risks
* Demonstrated ability to produce documents in a variety of accessible formats e.g. video, DVD, objects, for people with learning disabilities and their carers and also students with special needs and their education staff
Desirable
* Demonstrated ability to integrate Government legislation into care plans and individual Education, health and Social Care plans, as appropriate
* Demonstrated ability to provide second opinion to colleagues in area of clinical specialism
* Ability to understand and interpret specialist medical/information from other professionals and make these accessible to carers and people/ students with learning disabilities
