Palliative Care Clinical Nurse Specialist
Join our dynamic team on a fixed‑term contract as a Band 7 Palliative Care Clinical Nurse Specialist. If you are passionate about palliative care and curious yet unsure if the specialty is the right fit, this is your chance to work closely with the team and understand what it takes to be a Palliative Care CNS.
Key Responsibilities
* Provide expert palliative care nursing advice to support high‑quality, individualized care for patients and relatives/carers, in collaboration with the multi‑professional team.
* Manage your own caseload of patients, with appropriate input from Senior team, as part of the Specialist Palliative Care Team.
* Maintain effective communication with patients, families/carers and healthcare professionals to ensure seamless service delivery.
* Conduct comprehensive palliative care assessments, identifying and diagnosing patients’ physical, social, psychological, cultural and spiritual needs.
* Facilitate the provision of quality nursing care and contribute to decisions regarding clinical care and priorities.
* Attend or initiate case conferences as appropriate.
* Ensure patients are assessed, have care plans that are implemented and evaluated in accordance with Trust’s evidence‑based procedural documents.
* Act as a clinical expert for palliative and end‑of‑life care.
* Champion a culture of learning and quality improvement within the team.
* Be an ambassador for palliative and end‑of‑life care across the Trust.
