An exciting opportunity has arisen for an Advanced Nurse Practitioner within Annat Health Centre, Links Health Centre, Montrose.
The post holder will be an experienced advanced practice nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for unscheduled patients presenting to general practice.
The post holder will be part of a model of care undertaking telephone consultation and advice calls as well as participate in the daily house visits to patients.
The post holder will work with the wider practice team to delivery high quality care, with continuous evaluation and quality improvement by developing innovative concepts, models and practices to meet the changing needs of our practice.
You will work with the management team to develop services and implement tests of change aimed at realising the practice development plan and the RCGP 2022 vision for General Practice.
This is a new role within our practice and the post holder will recognise the challenges of delivering a new service to patients. The post-holder is a key member of the Annat multi disciplinary team, through which Advanced Nurse Practitioners will be required to deliver care that was previously provided only by GPs.
