Here at The Elms Medical Practice, we are looking to recruit a friendly and enthusiastic individual to support patients and clinicians as a care navigator/ receptionist. This is an exciting time of change across all GP practices and we are hoping to recruit someone with the right experience and attributes to help us implement new ways of working.
Main duties of the job
This practice has recently implemented a Total Triage model for triaging requests for help and allocating appointments. The ideal candidate would be expected to work alongside clinical and non-clinical staff to achieve this.
The role would include welcoming and helping patients to complete online consultations at our reception desk, as well as patients contacting the Practice via telephone and email. The role would also include triaging patients and booking appointments as well as advising patients about other local health and community services to meet their needs. Technology is fast becoming a useful way to contact patients and to automatically book clinics and appointments, including apps and text messaging services. This role would assist with embedding this new way of working, whilst learning how to use it to it's potential. The role would be based at our main site in Hoo and also at our branch site in Allhallows. There may also be a requirement to work at our other branch sites in High Halstow and Grain from time to time.
Hours
* Tuesday: 12pm to 5pm at our Hoo Site
* Wednesday: 10am to 6:30pm (with 30min unpaid break) at our Hoo Site
* Thursday: 12pm to 6pm at our Hoo Site
* Friday: AH 1pm to 6pm at our Allhallows site.

Essential
Good standard of general education
GCSE Mathematics C (or equivalent) or above
GCSE English C (or equivalent) or above
