Job Description:
Liquid Personnel is hiring a Social Worker for its client’s Front Door Team in Redbridge.
The role involves working as a care management social worker to assess the needs of vulnerable adults and their carers, and to arrange services that meet identified needs and outcomes. Promoting prevention, independence, self-directed support, choice, and control is central to the role. Key responsibilities also include risk management, safeguarding, and providing case management for adults requiring frequent specialist intervention.
What will your responsibilities be?
1. Conduct timely and proportionate initial assessments of service users and their carers to identify their needs and desired outcomes.
2. Provide immediate support and intervention to adults in crisis to stabilize their situation and prevent unnecessary hospital admissions.
3. Develop and implement individualized care plans that promote independence and recovery, linking service users to community-based services that enhance personal wellbeing.
4. Work as part of multi-agency teams to provide seamless health and social care services, ensuring effective communication and coordination.
5. Involve service users and carers in creative support planning, encouraging choice and control through the use of personal budgets, including Direct Payments as the default position.
6. Collaborate with Community Development Staff to help service users access community-based services and resources.
7. Adhere to risk management and adult safeguarding procedures in social work practice.
8. Conduct assessments and reviews of service users to a high standard, ensuring they reflect the needs and outcomes of the service user and carer.

Qualifications & Experience:
To be successful in this role you must have,
11. Social Work England registration
12. Eligible to work in the UK
13. Hold a full UK driving licence
14. Post-qualified experience
15. Degree level or equivalent in Social Work
